关节周围注射鸡尾酒式药物在人工膝关节置换术后镇痛研究 被引量:25

摘要 全膝关节置换术(TKA)是治疗严重膝关节疾病、矫正膝关节畸形、解除膝关节疼痛、改善膝关节功能的重要手段和有效方法,在全世界范围得到了广泛的认可和应用。然而,膝关节置换术后疼痛是人工关节手术无法回避的问题[1],通常会引起持续数天至数周的中、重度的疼痛,给关节置换术后患者带来痛苦、影响生活质量。关节置换术后疼痛还使患者术后应用膝关节及术后康复训练中产生忌惮,从而一定程度地影响了患者术后膝关节功能恢复。关节置换术后良好的疼痛控制对患者膝关节早期运动、离床活动和正常步态的恢复起重要作用[2]。患者对手术的成功与否及满意度也集中在手术能否减少疼痛与关节功能迅速恢复方面[3]。所以围术期镇痛的应用就显得格外重要。多模式镇痛是通过应用不同镇痛机制的药物,以不同的镇痛方式以获得更好的镇痛效果,并将并发症或不良反应降至最低。目前多模式镇痛在关节置换围术期得到了广泛的应用,其中包括手术部位局部注药混合镇痛药物,又称“鸡尾酒”疗法,是近几年来新出现的全膝关节置换围术期比较新颖的术后镇痛方法,具有较好的镇痛效果,可以较好地控制术后疼痛、减少不良反应和帮助患者康复[4,5]。
