摘要
目的 通过比较二尖瓣血流图结合肺静脉血流图法与彩色M型超声心动图基线移动法 ,探讨准确简便的左室舒张功能评价手段。方法 冠心病组 74人 ,按左室射血分数和E/A分为三组 :EF >5 0 %和E/A <1为A组 ,EF >5 0 %和E/A≥ 1为B组 ,EF≤ 5 0 %、E/A≥ 2为C组 ;对照组 2 2人在心尖四腔心切面测量E峰和A峰并计算E/A比值、E峰减速时间 (DT)、等容舒张时间 (IVRT)、彩色M型多普勒测量血流播散速率 (RFP) ,计算RFP/E。记录肺静脉血流图 ,测定S波、D波和其比值 (Vs/Vd) ,S波流速积分与S波、D波流速积分之和的比值FVTIs ,AR波的峰值 (ARV) ,AR波持续时间与A波持续时间的比值 (ARD/AD)。结果 冠心病组的三个亚组与对照组比较S波、FVTIs、ARV、ARD/AD、RFP、RFP/E都有显著差异 ,B组或C组的上述指标与A组都有显著差异 ,B组和C组间无显著差别。结论 两种方法都能鉴别二尖瓣血流图的伪正常 。
Objective The study was seeking for more exact and convenient way of detecting left ventricular diastolic function by pulmonary vein flow graph conbined with mitral valve flow graph compared with color m-mode echocardiography by baseline shift technique.Methods 74 patients suffered from coronary artery disease clssified by E/A ratio and LVEF as Group A(EF>50%and E/A<1),GroupB(EF>50%and E/A≥1)and Group C(EF≤50%and E/A≥2),were enrolled while 22 healthy persons treated as control group.E wave,A wave,E/A ratio,DT,IVRT,RFP,RFP/E,S wave,D wave,Vs/Vd,FVTIs,ARV and ARD/AD were detected in everyone.Result The value of S wavepeak velocity,FVTIs,ARV,ARD/AD,RFP,RFP/E were significantly different between control group and other three groups with coranary artere disease as well as between Group A and Group B or Group A and Group C(all P<0.01).There were no difference between Group B and Group C(p>0.05).Conclusion Both methods are capable in distinguish pseudonormalized transmitral flow pattern and evaluate precisely left ventricular diastolic function.
