摘要
目的分析电子输尿管软镜钬激光碎石术治疗复杂肾结石的临床效果。方法选取2010年3月～2012年7月本院收治的复杂肾结石患者共120例，依照入院顺序随机、平均分为治疗组和对照组，对照组给予开放性取石术治疗，治疗组给予电子输尿管软镜钬激光碎石术治疗，观察并记录两组的术中出血量、手术时间、结石清除效果、术后疼痛程度、术后发热率及下床活动时间、住院时间。结果治疗组术中出血量少于对照组、结石清除率高于对照组、手术时间短于对照组（P〈0．05）。治疗组患者术后发热、疼痛率低于对照组，下床活动时间及住院时间均明显短于对照组（P〈0．05）。两组均无大出血、脏器损伤、感染性休克等并发症。结论电子输尿管软镜钬激光碎石术治疗复杂肾结石相比传统开放性取石术用时短、结石清除率高、出血少、痛苦小、术后发热率低，下床活动及住院时间更短，安全有效。
Objective To analyze the clinical effect of electronic flexible ureteroscope holmium laser lithotripsy for the treatment of complex kidney stone.Methods 120 cases of patients with complex kidney stone in our hospital from March 2010 to July 2012 were selected and randomly divided into the treatment group and control group according to the order of admission,each group had 60 cases.The treatment of open stone surgery was given to the control group,elec- tronic flexible ureteroscope holmium laser lithotripsy was given to the treatment group.The amount of blood loss,opera- tive time,stone clearance effects,postoperative pain,postoperative fever rate and ambulation,hospitalization time in two groups was observed and recorded.Results The bleeding volume in the treatment group was lower than that in the con- trol group,the stone clearance rate in the treatment group was higher than that in the control group,the time of operation in the observation group was shorter than that in the control group （P〈0.05）.The incidence of fever and pain after surgery in the obse.rvation group was lower than that in the control group,the time of beginning out-of-bed activity and length of stay in the observation group were both shorter than those in the control group （P〈0.05）.There were no compli- cations of hematorrhea,organ injury and infectious shock in the two groups.Conclusion The operation of electronic flex- ible ureteroscope holmium laser lithotripsy for the treatment of complex kidney stone has the characters of shorter time, higher stone clearance rate,less bleeding,little pain,low incidence of fever after surgery,shorter time of beginning out- of-bed activity and length of stay compared to the traditional surgery of open stone surgery,and is safe and effective.
