摘要
目的对影响RICU中慢性阻塞性肺疾病急性加重期患者机械通气时间的因素进行分析。方法选择2012年9月至2013年8月入住郑州大学第一附属医院RICU中59例行有创机械通气的慢性阻塞性肺疾病急性加重期患者,分为长期机械通气组(≥14d)和短期机械通气组(≤7d),收集两组患者机械通气后第3天外周血中降钙素原(PCT)、白蛋白(ALB)、N-心房脑钠肽(BNP)、红细胞(RBC)、血红蛋白(HGB)等相关临床指标,并对其进行比较分析。结果两组患者外周血中PCT、BNP、ALB、RBC、HGB等临床指标在机械通气第3天比较差异均有统计学意义(P〈0.01)。结论肺部感染、心功能不全、低蛋白血症、贫血等会明显延长机械通气的时间,对其早期干预可缩短机械通气时间。
Objective To analyze the factors of influencing the time of using mechanical ventila- tion in patients with acute exacerbations of chonic obstructive pulmonary disease (AECOPD) in respiratory intensive care unit. Methods A total of 59 AECOPD patients hospitalized in the first affiliated hospital of Zhengzhou university from September 2012 to August 2013 were enrolled in this study. Fifty-nine patients were divided into two groups: twenty-eight subjects with short-time mechanical ventilation ( ≤ 7 days). Thirty-one with long-time mechanical ventilation( ≥14 days). Blood samples were collected from 59 AECOPD patients in the third day after mechanical ventilation. The levels of procalcitonin (PCT), albumin (ALB), N-type natriuretic peptide (BNP), red blood cell ( RBC ), hemoglobin ( HGB ) were compared between the two groups. Results The levels of PCT, BNP, ALB, RBC and HGB of peripheral blood on the third day after mechanical ventilation had significant differences (P 〈 0. 01 ). Conclusions Pulmonary infection, cardiac insufficiency, hypoproteinemia and anemia will prolong the time of mechanical ventilation in AECOPD patients significantly, and the early intervention of these factors will shorten the time of mechanical ventilation.
