摘要
目的探讨重型颅脑损伤术后患者预后现状和影响因素。方法查阅重型颅脑损伤术后患者相关文献制定自编问卷,应用于2010-07—2012-07在我院脑外科住院治疗的69例重型颅脑损伤术后患者进行临床资料的收集,根据患者格拉斯哥预后评分将其分为A组和B组,采用描述性统计和二分类Logistic回归分析重型颅脑损伤术后患者预后现状和影响因素。结果本研究入选的69例患者中预后良好率18.84%,预后不良率81.16%,多因素Logistic回归分析发现:凝血功能异常、S-100蛋白水平高、神经元特异性烯醇化酶水平高、脑干损伤和术后出现并发症是重型颅脑损伤术后影响患者预后的危险因素,而术后康复训练到位是影响重型颅脑损伤术后患者预后的保护因素。结论重型颅脑损伤术后患者预后总体欠佳,受多方面因素的影响,应针对性采取干预措施来提高患者预后,并加强康复训练改善患者预后。
Objective To investigate prognosis status and the influential factors of severe craniocerebral trauma. Methods The clinical data of sixty-nine patients with severe craniocerebral trauma in the department of neurosurgery in our hospital from July 2010 to July 2012 were collected by self-made questionnaire according to related literature of patients with severe cranioce- rebral trauma. Then they were divided into the group A and group B according to whether good prognosis or not, the collected clinical data were analysed by the SPSS16.0 for descriptive statistics and binary classification logistic regression analysis to find out prognosis status and its influential factors of it. Results The good prognosis rate was 18.84~ ,the bad prognosis was 81. 16 ~, and binary classification logistic regression analysis showed that coagulating dysfunction, high S-100 protein, high neuron specific enolase,brain stem injury and postoperative complications were risk factors, but postoperative rehabilitation training in place was a protective factor. Conclusion The prognosis status of severe craniocerebral trauma is bad,affected by various fac- tors. We should take measures to prevent severe craniocerebral trauma, strengthen the rehabilitation training to improve the prognosis of patients.
