摘要
目的规范危重患者院内转运程序,提高转运安全度,提升护士(尤其是低年资护士)的业务能力。方法采用回顾分析法,将存在的问题以任务清单式的理念设计出危重患者转运执行单,具体包括通知接收、全面评估、用物准备、环节确认、途中评估、接送交接等六项程序。结果护士按转运执行单指引,分步确认执行,弥补了低年资护士业务水平的个体差异,院内转运时间缩短、效率提高、风险降低。结论危重患者转运执行单操作性强,使用方便,使转运操作标准化、程序化、流程化,克服随意性和各种干扰因素,降低转运风险。
Objective Regularize the transfer procedure for critical patients and improve their transfer safety and nurses professional ability （especially for the junior nurses）. Methods Design transfer execution form based on the list of existing problems by means of retrospective analysis, including notification of admission, comprehensive evaluation, supplies preparation, link confirmation, halfway evaluation and transfer handover. Results With the guidance of the transfer execution form, nurses could conduct the procedure step by step, which largely make up the individual difference of professional ability among junior nurses, shorten the intrahospital transfer time, improve the efficiency and reduce the risks. Conclusion Transfer execution form for critical patients with high operability and convenience make the transfer procedure standardized, routinized and processized, which could overcome randomness and interference factors and reduce the transfer risks.
