摘要
目的:通过对患者的个体化药学服务,优化抗凝药物治疗,降低不良事件的发生,提高患者依从性。方法:结合临床药师的工作实践,总结对需接受抗凝治疗患者的药学服务内容和方法。结果:临床药师应根据患者不同的疾病状态、饮食因素和并用药物等方面,对华法林的治疗进行药学监护,并注重患者教育。结论:临床药师参与抗凝治疗的个体化药学服务,有利于促进抗凝药物的安全、合理使用。
OBJECTIVE To optimize anticoagulant therapy, reduce the risk of adverse events,and improve the compliance in patients by individualized pharmaceutical care. METHODS Active measures of pharmaceutical care for anticoagulant therapy were summarized by clinical pharmacists during pharmaceutical practice. RESULTS The clinical pharmacists carried out phar maceutical care and patients＇ education according to morbid state, diet and combined drugs. CONCLUSION The participation of clinical pharmacists makes drug anticoagulant therapy safer and more rational.
