摘要
目的初步探讨3 T MRI三维氢质子波谱成像(3D-1H-MRS)、扩散张量成像(DTI)对前列腺癌(PCa)的诊断价值。方法 23例PCa患者、19例良性前列腺增生(BPH)患者及20例健康志愿者均通过3D-1H-MRS及DTI检查,测得PCa、BPH、正常外周带(PZ)、正常中央区(CG)四组感兴趣区(ROI)内的平均(胆碱+肌酐)/枸橼酸盐(CC/C)值、表观扩散系数(ADC)值、各向异性分数(FA)值并生成相应的参数图,采用单因素方差分析获得四组CC/C、ADC、FA值的差异,并对PCa组的CC/C、ADC、FA值行Pearson相关性分析。结果平均CC/C值由高至低依次为PCa,BPH,CG,PZ,除PZ与CG外,余各组两两之间差异有统计学意义(P<0.05);平均ADC值由高到低依次为BPH,PZ,CG,PCa,除BPH与PZ外,余各组两两之间差异有统计学意义(P<0.05);平均FA值由高到低依次为PCa,CG,BPH,PZ,除PCa与CG外,余各组两两之间差异有统计学意义(P<0.05)。PCa组的CC/C、ADC值呈负相关(r=-0.457,P<0.01),CC/C、FA值呈正相关(r=0.380,P<0.05),ADC、FA值呈负相关(r=-0.400,P<0.05)。结论 PCa、BPH、正常前列腺的CC/C、ADC、FA值存在一定的差异,且PCa 3D-1H-MRS及DTI具有一定的相关性,两者结合对PCa的诊断具有广阔前景。
Objective To explore the diagnostic value of three dimentional proton MR spectroscopy (3D 1 H MRS) and diffusion tensor imaging(DTI) in the diagnosis of prostate cancer(PCa) at Philips 3T MRI. Methods 23 PCa patients, 19 benign prostatic hyperplasia(BPH) patients and 20 healthy volunteers underwent 3D^1H-MRS and DTI. The average val- ues of ( Choline + Creatinine)/Citrate ( CC/C ), apparent diffusion coefficients ( ADC ) and fractional anisotropy ( FA ) in PCa, BPH, normal peripheral zone (PZ) and central glands (CG), were measured, and related maps were generated. Single factor analysis of variance was used to analyze the differences of CC/C, ADC and FA value among four groups, and Pearson correlation analysis was used to study correlations betwwen CC/C, ADC and FA value. Results Average CC/C values from high to low were PCa, BPH, CG, PZ, each group had statistical differences except PZ and CG when compared in pairs (P 〈 0.05). Average ADC values from high to low were BPH, PZ, CG, PCa, each group had significant differences except BPH and PZ when compared in pairs(P 〈0.05). Average FA values from high to low were PCa, CG, BPH, PZ, there were significant differences between each group except PCa and CG when compared in pairs ( P 〈 0.05 ). In the PCa group ,a negative correlation was observed between the CC/C and ADC value( r =- 0. 457 ,P 〈 0. O1 ) , a positive correla- tion was observed between the FA and CC/C value( r = O. 380, P 〈 0. 05 ) , a negative correlation was observed between the FA and ADC value( r = - 0. 400, P 〈 0.05 ). Conclusion There are certain differences between PCa, BPH and normal prostate in CC/C,ADC,FA value. 3D ^1H-MRS and DTI of PCa have some relevance, the combination of them has broad prospects in the diagnosis of PCa.
