摘要
目的探讨DWI表观扩散系数(ADC)值与乳腺浸润性导管癌分子预后标记物间的相关性。方法回顾性分析82例经手术或活检证实的乳腺浸润性导管癌患者的DWI(b=0,1000 s/mm2)资料,测量ADC值;免疫组织化学染色记录分子预后标记物ER、PR、Ki67、HER2的表达情况,用非参数K-W检验、Mann-Whitney U检验及Spearman等级相关分析比较不同核级别浸润性导管癌间的ADC值差异及不同分子预后标记物与ADC值间的相关性。结果不同级别的浸润性导管癌的ADC值间存在差异(χ2=11.160,P=0.04)。ADC值与ER表达强度(χ2=9.401,P=0.024)、Ki67表达(χ2=7.306,P=0.026)强度间存在相关性,且ADC值与二者表达百分比间存在弱负相关(r=-0.346,r=-0.289)。ADC值与PR表达(χ2=3.499,P=0.321)、HER2表达(χ2=-1.467,P=0.143)及三阴乳腺癌与非三阴乳腺癌间表达无相关性(χ2=-0.258,P=0.796)。结论低ADC值与高核级别及ER、Ki67高表达相关,而与HER2、PR表达无相关性,反映了乳腺癌不均质性的生物学特性,可能对患者的治疗方式及预后提供有价值的信息。
Objective to evaluate the relationship of the apparent diffusion coefficient (ADC) value of breast cancer with prognostic factors. Methods 82 patients with breast cancer underwent MRI examination and diffusion weighted ima- ging. ADC value was calculated using two b factors (0 and 1000 s/mm- 2). ADC values of invasive ductal carcinoma (IDC) and immunochemical prognostic factors were compared with the Kruskal Wallis test, Mann Whitney U test and Spearman.. Results There was significant difference ( X2 = 11. 160, P = 0.04) of ADC value among different histological grade of IDC. There was correlation of ADC value with estrogen receptor (ER) and Ki67, and ADC value had poor nega- tive correlation with expression percentag. No correlations were observed between ADC value and human growth factor re- ceptor 2 expression (HER2) ( X2 = - 1. 467,P = 0. 143 ), progesterone receptor(PR) ( X2 = 3. 499,P = 0.321 ) and triple negative breast carcinoma(TNBC) ( X2 = -0. 258,P = 0. 796 ). Conclusion Low ADC value was correlated with positive expression of ER, Ki 67 index, which reflected the inhomogeneity of breast cancer and might provide valuable information for the treatment and prognosis.
