液基薄层法与细胞DNA倍体分析在宫颈癌筛查中的应用 被引量:5

摘要 目的探讨细胞DNA定量分析法与液基薄层细胞学在宫颈癌及癌前病变诊断中联合检测的作用。方法对本院妇科就诊的2 459名妇女进行子宫颈癌筛查,用宫颈刷取材,进行液基薄层制片,同时做液基薄层细胞学检查和全自动DNA倍体分析系统的DNA定量测定,对检查可疑的妇女在阴道镜下行宫颈组织学活检,并将其结果作为诊断标准。结果液基薄层细胞学检查发现214例(8.01%)异常,包括156例(5.84%)ASCUS、45例(1.68%)LSIL和11例(0.41%)HSIL。DNA倍体分析系统发现105例(3.93%)异常。161例妇女行宫颈组织学活检发现:5例癌、11例CINⅢ/CIS、18例CINⅡ。以病理诊断结果为标准,计算DNA定量分析方法在筛查CINⅢ以上宫颈病变的敏感度为58.8%,特异性为87.5%;液基薄层细胞学将LSIL定为筛查需做活检的标准,液基薄层细胞学筛查CINⅢ以上的宫颈病变的敏感性为56.2%,特异性为85.5%;将HSIL定为筛查需做活检标准,液基薄层细胞学筛查CINⅢ以上的宫颈病变的敏感性为37.5%,特异性为95.8%。结论用液基薄层细胞学和细胞DNA定量分析联合检测可明显提高宫颈病变的阳性检出率,减少漏检率。 Objective To explore the function of cell DNA quantitative analysis method and liquid based thin-layer cytology for the diagnosis of cervical cancer and precancerous lesions.Methods 2 459 women were collected from clinic gynaecology for the cervical cancer screening,samples were harvested with cervical brush to prepare thin liquid layer and,simultaneously,liquid thin layer cytology examination and full automatic DNA ploidy quantitative measurements were carried out by using cervical biopsy under colposcope.Results 214 cases of abnormalities (8.01%) were identified with the cytology examination,including 156 cases of ASCUS,45 (1.68%)of LSIL (5.84%),and 11 cases (0.41%)of HSIL.DNA ploidy analysis revealed 105 cases (3.93%).One hundred and sixty-one women were subjected to cervical histology biopsy,and 5 cases of carcinoma,11 cases of CIN Ⅲ/CIS,18 cases of CIN Ⅱ were comfirmed by pathological identification.The DNA quantitative analysis gave rise to a sensitivity of 58.8% and specificity of 87.5% in screening CIN Ⅲ.Biopsy should be given to those patients with LSIL based on the liquid thin-layer cytology examination as a screening method.The liquid thin layer cytology screening for CIN Ⅲ cervical lesions showed a sensitivity of 56.2% and specificity of 85.5%.Compared to the biopsy examination of HSIL,liquid thin-layer cytology for screening of CIN Ⅲ cervical lesions exhibited a sensitivity of 37.5% and specificity of 95.8%.Conclusion Using combination of liquid thin-layer cytology and cell DNA quantitative analysis will improve the detection rate of cervical lesions.
