摘要
目的调查本地区人群血小板抗原(HPA)系统等位基因的分布频率,并进行随机输血风险评估。方法随机收集无偿献血者样本400份,应用PCR-SSP技术检测血小板抗原的基因型。结果所有样本中均含有高频基因"a"、"aa"、"ab"、"bb"三种基因,在HPA-3和HPA-15中所占比例的差异无统计学意义,但在所检测的其他抗原中分布不均,各种抗原引起随机输血不合概率的差异有统计学意义。结论 HPA-3和HPA-15是引起洛阳地区血小板输注不合的主要原因,基因鉴定技术可为血小板输注无效患者提供HPA相合的血小板。
Objective To investigate the distribution of the major antigens in HPA system in Luoyang,and establish a suitable method to genotype the human platelet antigens.Methods Four hundreds of samples were collected randomly.The PCR-SSP technique was applied to genotype the human platelet antigens.Results The high frequency gene " a" was found in all samples.Except for HPA-3 and HPA-15,the distributions of "aa","ab" and "bb" had significant difference,and the mismatch probabilities in different antigens were also significantly different.Conclusion The HPA-3 and HPA-15 were the major reason for the mismatch of platelet in Luoyang,and the gene identification technique can provide the patients with paltelet transfusion refractoriness with HPA-matched platelet.
