红细胞分布宽度在肺栓塞诊断中的应用价值 被引量:11

摘要 目的探讨红细胞分布宽度(RDW)在诊断肺栓塞(PE)中的价值。方法采用回顾性方法,分析PE(n=61)和非PE(n=75)两组患者的临床资料,比较两组RDW、D-二聚体、N末端B型利钠肽前体、肌钙蛋白I、红细胞计数、血小板计数、血红蛋白、纤维蛋白原、血肌酐、低密度脂蛋白胆固醇、C反应蛋白等多项指标的差异。结果 PE组RDW>14.6%者占37.7%,两组RDW>14.6%者比例比较,差异有统计学意义(P=0.036)。两组间深静脉血栓者比例比较差异有统计学意义(P=0.046);两组间恶性肿瘤和骨折手术者比例比较,差异无统计学意义(P>0.05)。纤维蛋白原、肌钙蛋白I、D-二聚体和N末端B型利钠肽前体在两组间的差异也有统计学意义(P<0.05)。单因素logistic回归结果显示,RDW>14.6%为PE诊断的危险因素。多因素logistic回归分析未发现有对PE诊断有意义的危险因素。结论 RDW升高与PE相关,对辅助早期鉴别PE患者具有一定意义。
