摘要
脉管畸形是脉管系统发育时错构形成的紊乱畸形,依据其主要病变成分可分为微血管畸形、静脉畸形、动静脉畸形、淋巴管畸形以及混合畸形。多数脉管畸形在儿童期即可明确诊断。这一时期如果处理得当,病变的快速生长及后期并发症可得到有效控制和预防;如果处理不当,可能带来后期难以处理的并发症及功能损害。儿童脉管畸形患者尚处于生理发育期,病变的临床特点、生物学行为、发生发展都有其特殊性。在这一时期制定治疗方案时应从整体性、长期性的角度考虑。在有效消除病变的同时,也要考虑治疗方法对患者组织发育的影响,对患者颜面外观的影响,对面部表情、言语、吞咽、咀嚼等功能的影响;同时,也要考虑治疗对患者心肺系统、肝肾功能的影响,评价患者对治疗的耐受程度。全局化的考虑有助于医生制定科学、合理的治疗方案,在取得理想疗效的同时使不良反应发生率降到最低。
Vascular malformations are disorders representing errors in vascular development. According to the predominant type of vessel, vascular malformations are classified as capillary malformation, venous malformation, arteriovenous malformation, lymphatic malformation or combined. Most vascular malformations can be diagnosed in childhood, and if they are properly handled during this period, the rapid expan- sion and later complications may be prevented ; if not it may lead to the complications and damage of the function which are very- difficult to deal with. Children with vascular malformations are in the period of physiological growth. The clinical features, biological behavior and de- velopment of vascular malformations at this stage are of particularity. Making the treatment plan during this period should be from a view of entirety and long-term management. While effectively eliminate lesions, we should also consider the impact of treatment on the patient~ tis- sue development, cervicofacial appearance, function of facial expression, speech, swallowing and chewing. Furthermore, we must also con- sider the impact of treatment on cardiopulmonary system, function of liver and kidney. The children~ tolerance to the treatment should also be evaluated. A global-oriented view helps doctors develop a scientific and rational treatment plan, to achieve the desired therapeutic effect and minimize the complications.
