深圳市儿童医院急性呼吸道感染患儿人冠状病毒感染情况分析 被引量:5

Detection of human coronaviruses in children with acute respiratory tract infection in Shenzhen Children's Hospital

摘要 目的了解2009年6月—2010年5月深圳市儿童医院急性呼吸道感染患儿人冠状病毒(human coronavirus,HCoV)-OC43、HCoV-229E、HCoV-NL63、HCoV-HKU1感染情况及临床特征。方法呼吸道感染患儿533例,采集鼻咽拭子标本后应用实时荧光定量PCR方法检测人HCoV-OC43、HCoV-229E、HCoV-NL63、HCoV-HKU1,分析HCoV感染患儿临床特征、感染情况及季节分布等特点。结果 HCoV感染患儿29例,感染率为5.4%,其中HCoV-OC43感染16例,HCoV-229E感染4例,HCoV-NL63感染6例,HCoV-OC43及HCoV-229E混合感染3例;27例(93.1%)患儿年龄≤3岁;感染率居前4位的月份依次为1月(11.9%)、9月(8.1%)、10月(7.1%)、11月(7.1%);临床表现为发热、咳嗽、气喘,6例合并哮喘急性发作,诊断为支气管肺炎16例、急性支气管炎11例、毛细支气管炎1例、急性上呼吸道感染并轮状病毒肠炎1例。结论急性呼吸道感染患儿HCoV感染高峰在秋冬季,以HCoV-OC43感染多见,年龄≤3岁儿童易感。 Objective To acquire the epidemiological and virologic characteristics of human coronavirus (HCoV)-OC43, -229E, -NL63, and -HKU1 in hospitalized children with acute respiratory tract infection (ARTI) in Shenzhen Children's Hospital from June 2009 to May 2010. Methods A total of 533 children with ARTI were collected nasopharyngeal swabs and were detected HCoV-OC43, HCoV-229E, HCoV-NL63 and HCoV-HKU1 by real-time PCR to analyze clinical features, infection condition and seasonal distribution. Results HCoV infection was detected in 29 patients (5. 4%), including HCoV-OC43 infection in 16 patients, HCoV-229E infection in 4 patients, HCoV-NL63 infection in 6 patients, and mixed infection of HCoV-OC43 and HCoV-229E in 3 patients, in which 27 patients (93. 1%) were 3 years old or younger. HCoV infection was mostly epidemic in January (11. 9~%00), September (8. 1%), October (7. 1%) and November (7.1%) in turn. The clinical presentations of HCoV infection included fever, cough and shortness of breath. Six patients were complicated with acute asthma. Sixteen patients were diagnosed pneumonia, 11 were diagnosed acute bronchitis, 1 patient was diagnosed bronchiolitis, and 1 patient was diagnosed acute upper respiratory infection complicated with rotavirus enteritis. Conclusions The peak of prevalence of HCoV is in autumn and winter in ARTI children. HCoV-OC43 infection is the most commonly found and the children aged 3 years or younger are prone to be infected.
