摘要
目的 了解新生儿完全性大动脉转位 (TGA)的超声特点 ,评价超声在TGA早期诊断中的价值。方法 回顾分析 35例新生儿TGA的超声表现并与手术、心导管和尸解结果对照。结果 单纯TGA 6例 ,合并室间隔缺损、房间隔缺损、动脉导管未闭 (PDA) 2 0例 ,房室间隔缺损 1例 ,肺动脉狭窄 1例 ,肺静脉畸形引流 1例 ,三尖瓣闭锁 2例 ,右位心 2例 ,冠状动脉异常 1例。合并 3种以上其他先心病共 13例。大动脉位置关系与 4例手术、1例心导管、2例尸解结果一致。 1例PDA彩超漏诊。结论 新生儿TGA需早期诊断 ,超声检查无创。
Objective To evaluate echocardiography in early diagnosis by studying echocardiography features of TGA in newborns. Methods 35 cases of neonatal TGA were analyzed retrospectively and compared with the findings of operation, catheterization and autopsy. Results Among the 35 cases,20 cases were with ventricular septal defect(VSD),atrial septal defect(ASD) and patent ductus arteriosus(PDA).1 case with endocardial cushion defect(ECD) ,1 case with pulmonary valve stenosis(PS),1 case with total anomalous pulmonary venous connection(TAPVC),2 cases with tricuspid atresia(TA) and 1 case with anomalous origin of coronary artery.13 cases were with more than three kinds of other congenital heart diseases. Echocardiography showed the changes of the great arteries were compatible with those seen under 4 cases of operation,1 case of catheterization and 2 cases of autopsy.1 case of PDA was missed by Echocardiography. Conclusion TGA in newborns should be early diagnosed. Echocardiography is an invasive and reliable technique and it can provide accurate informations for diagnosis of newborns with TGA.
