树突状细胞及调节性T细胞与慢性丙型肝炎发病机制的关系 被引量:3

摘要 全球大约有1.7亿人发生慢性丙型肝炎病毒(HCV)感染。HCV感染自发性清除受到多种因素的影响,包括年龄、种族、性别、HBV或HIV共感染,以及宿主的遗传学(如HLA单体型和IL28B基因变异)。尽管在 HCV 感染初期都检测到一系列免疫反应,但最终发展为慢性感染的个体都存在免疫抑制的情况。自发性清除 HCV感染的个体持续呈现出对HLA Ⅰ类(CD8+)和Ⅱ类(CD4+)限制性T 细胞的增殖反应,而慢性感染者仅有少量增殖反应。对于慢性感染者,多种因素可导致T细胞反应的降低,如病毒突变,逃逸CD4和CD8 T细胞的识别、CD4+T细胞的无反应、CD8+T细胞的耗竭、转录因子叉头框蛋白(FoxP3+)调节性 T(reg)细胞的诱导和(或)树突状细胞功能的损害。
