摘要
目的 探讨血糖浓度对普罗帕酮转复阵发性心房颤动（房颤）患者疗效的影响.方法 回顾性分析2010年1月至2012年12月在复旦大学附属中山医院青浦分院住院患者中的阵发性房颤（房颤持续时间＜24 h）108例的临床资料.依据患者就诊时的血糖浓度分为3组：Ⅰ组（血糖浓度＜7.0 mmol/L）52例;Ⅱ组（血糖浓度7.0～11.0 mmol/L） 33例;Ⅲ组（血糖浓度≥11.1 mmol/L）23例.所有患者转复前完成糖化血红蛋白检测、超声心动图检查,采用盐酸普罗帕酮药物转复.结果 Ⅰ、Ⅱ两组转复率分别为94.2％（49/52）、84.8％（28/33）,与Ⅲ组69.6％（16/23）比较,差异有统计学意义（P＜0.05）.Ⅰ、Ⅱ两组糖化血红蛋白水平明显低于Ⅲ组,差异有统计学意义（P＜0.05）.各组左心房内径比较,差异无统计学意义（P＞0.05）.结论 血糖浓度≥11.1 mmol/L的阵发性房颤患者使用普罗帕酮的转复率低.
Objectives To investigate the effect of blood glucose level on patients treated with propafenone for paroxysmal atrial fibrillation.Methods In the retrospective study,108 patients with paroxysmal atrial fibrillation （duration＜24 h） admitted to Qingpu Medical Center,Zhongshan Hospital from January 2010 to December 2012 were included.According to the level of blood glucose,patients were divided into 3 groups：group Ⅰ （blood glucose level＜7.0 mmol/L,52 cases）,group Ⅱ （blood glucose level among 7.0-11.0 mmol/L,33 cases） and group Ⅲ （blood glucose level ≥ 11.1 mmol/L,23 cases）.The glycosylated hemoglobin （HbA1c） and echocardiography were examined before pharmacological cardioversion with propafenone.Results Success rates of cardioversion with propafenone in group Ⅰ and group Ⅱ were 94.2％ （49/52） and 84.8％ （28/33）,respectively,and when compared with that of group Ⅲ [69.6％ （16/23）],the difference was significant （P＜0.05）.Levels of glycosylated hemoglobin （HbA1c） in groups Ⅰ and Ⅱ were significantly lower than that in group Ⅲ （P＜0.05）.There was no significant difference in left atrial dimension among the three groups （P＞0.05）.Conclusions When blood glucose level ≥ 11.1 mmol/L,the success rate of cardioversion with propafenone is low in patients with paroxysmal atrial fibrillation.
