摘要
目的 调查心内科住院患者心房颤动病因,并与1996年的调查结果进行对照分析,探讨近年来住院患者心房颤动病因变化趋势.方法 回顾性分析南方医院心内科近5年的全部住院病历资料,剔除重复住院患者,其中心房颤动患者1 316例(占总住院人数10.9%).结果 与1996年的调查结果比较,心房颤动患者平均年龄呈增长趋势(63岁vs.46.5岁),风湿性心脏病合并心房颤动比例明显下降(18.8% vs.73.6%),原发性高血压(高血压)及特发性心房颤动较以往更为多见,分别占28.4% 、20.6%.不同年龄组心房颤动病因构成差别显著:低年龄组(<40岁)以特发性及风湿性心脏病心房颤动最常见,分别占38.4%、35.6%;中龄组(40~60岁)以风湿性心脏病及特发性心房颤动最常见,分别占30.7%、19.8%;高龄组(>60岁)以高血压及特发性心房颤动最常见,分别占37.5%、19.4%.结论 随着心血管疾病谱的改变,住院患者心房颤动病因构成比有明显变化,风湿性心脏病心房颤动比例呈明显下降趋势,高血压心房颤动、特发性心房颤动呈增加趋势;不同年龄组,心房颤动构成比亦有明显不同.
Objectives To investigate the pathogenesis of atrial fibrillation (AF) in patients from the Department of Cardiology and compare to the findings of 1996 to explore the variation tendency.Methods We retrospectively reviewed all the patients who were hospitalized in Department of Cardiology in the last five years and excluded the repeated ones,and thus 1 316 patients with AF were included(10.9% of all the patients).Results Compared with 1996,the average age of patients with AF exhibited a growing trend (63 years vs.46.5 years).The ratio of rheumatic heart disease related with AF obviously declined (18.8% vs.73.6%),however,hypertension related with AF and idiopathic AF increased (28.4% and 20.6% respectively).Idiopathic AF,and rheumatic heart disease related with AF were mostly in the low age group (< 40 years old),occuping 38.4% and 35.6% respectively,and also mostly in the middle age group (40-60 years old),respectively occupying 19.8%,30.7%.However,hypertension and idiopathic AF were mostly in the old age group(>60 years old,respectively occupying 37.5%,19.4%.Conclusions Along with the change of spectrum of cardiovascular disease,the pathogenesis constituent ratio of patients with AF has changed obviously.The ratio of rheumatic heart disease with AF is obviously descended,however,hypertension related with AF and idiopathic AF increased.The pathogenesis of AF also differed among age groups.
