摘要
目的:探讨经皮内镜下胃造口管(PEG)在克罗恩病(CD)病人肠内营养(EN)支持中的应用效果及与传统鼻胃管相比的优越性。方法:回顾性分析2009年1月至2012年1月在我科接受管饲EN的CD病人的临床资料,比较不同管饲途径(鼻胃管与PEG管)在治疗中的耐受情况和并发症的发生情况。结果:EN支持期间PEG组并发症发生率显著低于鼻胃管组(23.1%vs 52.6%,P<0.05)。至第4周开始,鼻胃管组累积并发症的发生率明显高于PEG组(36.8%vs 11.5%,P<0.05)。结论:PEG在CD病人营养支持中的应用安全、可靠、并发症少,可作为CD病人长期EN支持的管饲途径。
Objective: Our retrospective study was aimed to investigate the safety and feasibility of percutaneous endoscopic gastrostomy (PEG). Methods : Clinical data of patients underwent enteral nutrition for CD from 2009 to 2012 was retrospectively analyzed. Tube complications in patients with PEG were compared with those in patients with nasogastric feeding tube (NGT). Results : A total of 64 patients were analysed, in which 26 patients were under PEG and 38 patients received nasogastric feeding tube. The tube complication rates was significantly lower in PEG group than that in NGT (23. 1% vs 52.6% ; P =0. 021 ) and the significant difference began at 4 weeks of EN( 11.5% vs 36.8%, Log-rank P = 0. 041 ). Conclusion: PEG is a convenient, safe and less complication method for CD patients who need to be treated with a long-term enteral nutrition.
