高血压病患者认知功能与炎症的相关性研究

Study on the relationship between cognitive function and inflammation in hypertensive patients

摘要 目的探讨高血压病患者认知功能损害与炎症水平的关系。方法收集到我院就诊的200例高血压病患者,根据国际通用的简易智力状况量表(mini mental state examination,MMSE)得分进行认知功能评定。将患者分为两组:认知功能障碍组作为病例组44例,认知功能正常组作为对照组156例。按统一方法测量血压、身高、体重等,收集空腹静脉血,测定血脂,血糖,胰岛素,超敏C反应蛋白(hs-CRP),肿瘤坏死因子-α(TNF-α)等。用胰岛素抵抗指数(IRI)作为胰岛素敏感性评价指标。结果高血压病患者的文化程度、饮酒史、脑血管病史、年龄、舒张压、胰岛素、IRI,hs-CRP及TNF-α在两组间的差异有统计学意义(P<0.05)。非条件Logistic多因素回归分析显示:年龄、文化程度、脑血管病史、IRI,hs-CRP及TNF-α与认知功能成正相关(OR值分别为1.097、7.141、4.012、1.018、1.919、1.125,P<0.05)。结论高血压病患者认知功能损害除与文化程度、年龄因素及脑血管病史相关外,还与炎症及胰岛素抵抗密切相关。炎症是高血压病患者认知功能障碍重要的危险因素。 Objective To explore the relationship between cognitive function and inflammation in hypertensive patients. Methods 200 hypertensive patients were included. We usd the Mini Mental State Examination (MMSE) to test the cognitive function and calculate scores. Primary hypertensive patients were divided into two groups: the patients with impaired cognition were classified as case group (n =44), with normal cognition as control group(n = 156). The basic information was collected. Blood pressure, height and body weight were recorded. Total cholesterol, triglyceride, fasting plasma glucose, fasting plasma in- sulin, serum hs-CRP and TNF-c~ level were measured. Insulin resistance index(IRI) was calculated to estimate insulin resistance. Results Univariate analysis showed that significant difference were found in age, educational level, history of cerebrovascular disease, insulin resistance index, hs-CRP and TNF-ot between two groups. Multi-variate logistic regression analysis in 200 patients showed that age, educational level, history of cerebrovascular disease, IRI, hs-CRP and TNF-a were correlated to cognitive impair- ment. Conclusion Besides educational level, age and history of cerebrovascular disease, insulin resistance index and inflamma- tion are associated with cognitive impairment. Inflammation may be an independent risk factor of cognitive deterioration in primary hypertensive patients.
