摘要
目的 检测胃癌组织中细胞早期凋亡的发生情况 ,探讨其与CT扫描结合对判断预后的意义。方法 收集 41例胃癌及癌旁新鲜组织 1.0cm3 大小 ,分离单细胞悬液后 ,流式细胞术检测 ,AV/PI双参数分析细胞早期凋亡指数 (EAI) ;测量胃癌CT扫描中肿瘤组织的范围和厚度。结果 流式细胞术AV/PI双参数法可以明确地将单细胞悬液区分为凋亡细胞、坏死细胞和活细胞。其中 ,肿瘤组织的EAI明显高于瘤旁组织 (16 .1%± 4.8%vs9 6 %± 3.6 % ,P <0 .0 1) ,EAI与肿瘤组织类型和有无淋巴结转移有显著关系 (P <0 .0 1) ;CT扫描肿瘤范围和厚度与肿瘤的病理类型关系不密切 ,但从CT分型的浸润型胃癌厚度超过 2 0mm时 ,其与EAI呈正相关 (r =0 .5 8,P <0 .0 5 )。结论 胃癌组织的EAI在不同的组织类型和有无淋巴结转移方面不同 。
Objective To explore the prognostic value of combined early apoptotic index(EAI) and CT findings in patients with gastric carcinoma.?Methods Forty-one fresh specimens of gastric carcinomatous and adjacent non-neoplastic tissues of 1.0 cm 3 were collected and processed into single-cell suspension to assess EAI in AV/PI bivariate by using flow cytometry (FCM). The dimensions of tumor were determined by CT.?Results The suspending cells incubated with AV and PI resulted in 3 distinct cell populations (apoptotic, necrotic and living cells) to be identified by FCM. The EAI of vicious tissues was higher than that of adjacent tissues (16.1%±4.8% vs 9.6%±3.6%, P <0.01). EAI varied markedly with Lauren classification ( P <0.01) and lymph node metastasis ( P <0.01). The size of tumor was not related to its histopathology, but when the tumor was more than 20 mm in depth under CT, the size was positively correlated to EAI( r =0.58,P<0.05).?Conclusion The EAI of gastric carcinoma varies significantly with difference in Lauren′s classification and presence of lymph node metastasis. The combination of EAI and CT may help in assessing therapeutic effect and predicting prognosis of the gastric carcinoma.
