急诊急性左心衰无创呼吸机辅助通气与高氧吸入的对比研究 被引量:7

Comparison of the Clinical Effects between Noinvasive Ventilation and High Oxygen Inhalation on the Treatment of Acute Left Heart Failure

摘要 目的:对比无创呼吸机辅助通气与高氧吸入方法治疗急性左心衰的临床疗效。方法:选取我院急诊科于2011年3月至2012年12月收治的急性心衰患者80例,随机分为两组。观察组采用无创呼吸机辅助通气治疗,对照组采用高氧吸入治疗。观察并比较两组患者的心率、呼吸系统改善情况、血压波动情况、心功能变化、血气分析等相关指标的变化情况。结果:观察组治疗的有效率72.50%,对照组为52.50%,观察组明显优于对照组,差异有统计学意义(Ridit z=15.328,P=0.000);治疗后,观察组患者的RR、SBP、DBP、心率、心功能变指数及左室射血分数(Left Ventricular Ejection Fraction,LVEF)等相关指标均显著优于对照组,差异有统计学意义(P<0.05);血气分析PaO2差异有统计学意义(P<0.05);PaCO2和SaO2差异无统计学意义(P>0.05)。结论:无创呼吸机辅助通气可以改善患者临床症状体征和血气指标,改善患者心脏功能,对急诊急性左心衰疗效显著。 Objective: To compare the clinical efficiency between noninvasive ventilation therapy and high oxygen inhalation on the treatment of the acute left heart failure. Methods: 80 patients with the acute left heart failure .who were selected and randomly divided into two groups. The patients in the observation group were treated by the noninvasive ventilation therapy, while the patients in the control group were treated by the high oxygen inhalation therapy. Then the heart rate, respiratory, blood pressure, heart fimction, blood gas analysis and curative effect of patients were observed and compared between two groups. Results: In the observation group, the effective rate was 72.50%, which was higher than that of the control group with statistically significant difference (Ridit z=15.328, P=0. 000); After treatment, there were statistically significant differences between the two groups in terms of RR, SBP, DBP, heart rate, TEl index and LVEF of the patients (P〈0.05); There was statistically significant difference about the blood gas analysis of PaO2 (P〈0.05); There was no significant difference between PaCO~ and SaO2 (P〉0.05). Conclusion: Noninvasive ventilation therapy can improve clinical symptoms and blood gas index, improve heart function of patients, which has significant therapeutic effect on acute left heart failure.
