2型糖尿病患者胰岛β细胞功能衰竭与脂联素和抵抗素的临床相关研究 被引量:15

Relationship of serum adiponectin and resistin with pancreatic β cell failure in type 2 diabetes

摘要 目的探讨T2DM患者胰岛β细胞功能衰竭所致APN和抵抗素的变化特点及临床意义。方法选取T2DM病程<6个月者21例,6个月<T2DM病程<2年者20例,T2DM病程>2年者18例及健康对照(NC)者21名。测量身高、体重、BP和WC,检测真胰岛素(TI)、胰岛素原(PI)、APN、抵抗素、FPG、FIns、TG、TC、HDL-C和LDL-C,计算胰岛β细胞功能指数(HOMA-β)。结果随着T2DM病程进展,胰岛β细胞功能进行性下降,TI分泌逐渐减少(P<0.05)。T2DM病程较长组PI高于病程较短及NC组(P<0.05)。T2DM各组TG、TC、LDL-C、APN和抵抗素与NC组比较差异有统计学意义(P<0.05)。结论随着T2DM病程进展,胰岛β细胞功能逐渐衰竭,且伴随血浆APN和抵抗素水平改变,这可能是导致T2DM发病原因之一。 Objective To investigate the relationship of serum adiponectin and resistin with pancreatic β cell dysfunction and their clinical significance in T2DM patients. Methods T2DM patients with coursetwo years in 18 cases and normal control (NC) in 21 cases were selected. Height, weight, blood pressure, WC, serum concentration of true insulin (TI), proinsulin (PI), adiponectin, resistin, FPG, FIns, TG, TC, HDL-C, LDL-C were detected and HOMA-β was calculated. Results The HOMA-βwhich reflects the secretory function of pancreatic β cell declined significantly with the progress of T2DM(P〈0.05). TI and PI were significantly higher in long course T2DM group than in short course T2DM and NC group (P〈0.05). The serum resistin,TG, TC and LDL-C were significantly increased in each T2DM group than in NC group (P〈0.05). Conclusion Along with the progress of T2DM and decreased secretory function of pancreatic β cell, serum adiponectin declines , and serum resistin increases gradually and significantly which may correlate with T2DM development.
