摘要
目的探讨心理因素对胃癌患者中医证型的影响。方法运用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)对295例胃癌患者进行心理测评。观察脾虚证、气血亏虚证、气阴两虚证、脾胃阴虚证及肝胃不和证5种中医证型中焦虑、抑郁情绪的发生情况。结果 (1)气血亏虚证、脾虚证胃癌患者焦虑、抑郁情绪的发生率较高(57.6%、40.7%);(2)胃癌患者各证型的HAMA和HAMD评分差异有统计学意义(F=11.51、12.46,P<0.01)。2种评分均以气阴两虚证最高,HAMA评分以肝胃不和证次之,HAMD则以气血亏虚证次之。结论焦虑、抑郁等心理因素对胃癌中医证型有一定影响。
Objective To explore the influence of psychological factors on TCM Syndrome types of gastric carcinoma. Methods 295 patients with gastric carcinoma were evaluated with Hamilton Depression Rating Scale （HAMD） and Hamilton Anxiety Scale （HAMA）, and we observed the difference of depression and anxiety between the different groups of TCM Syndrome type. Results （1） The occurrence rate of syndrome of of qi and blood deficiency, and syndrome of spleen deficiency was higher than others （57.6% ,40. 7%）;（2） There was a significant difference in different syndrome structure in the score of HAMD and HAMA of patients （F = 11.51,12.46, P 〈0,01）. The score in syndrome of qi and yin deficiency group was the highest in all the two scales. But the syndrome of incoordination between liver and stomach was higher in HAMA, and the syndrome of qi and blood deficiency was higher in HAMD. Conclusion Psychological factors like anxiety and depression plays an important role in TCM Syndrome types of gastric carcinoma.
