经椎旁肌入路在治疗胸腰段椎体骨折中的应用 被引量:2

摘要 胸腰段骨折是最常见的脊柱骨折类型。对于需行手术者,后路切开复位椎弓根内固定手术是最常用的方法。常规后路手术中常需广泛剥离椎旁肌以获得良好的手术视野,然而对椎旁肌大面积的剥离和长时间牵拉,致使椎旁肌缺血坏死和失神经支配,往往导致顽固性腰背痛、腰背肌无力等并发症的发生。为了减少手术对椎旁肌的损伤以及减少并发症的发生,wiltse等于1968年提出经多裂肌和最长肌间隙入路来替代传统后正中入路用于腰椎的后外侧融合和不需椎管减压的椎弓根置钉。我们应用Wiltse人路即椎旁肌入路治疗无需椎管减压、单节段的胸腰段椎体骨折,并与传统后正中人路进行了对比研究,现报告如下。
