同期房颤双极射频消融对二尖瓣机械瓣膜置换术疗效的影响 被引量：14

摘要 目的 评估同期房颤射频消融对二尖瓣机械瓣膜置换术患者疗效的影响.方法 48例风湿性二尖瓣病变合并心房纤颤患者被纳入研究.根据是否同期进行房颤双极射频消融术分为房颤双极射频消融组（n=26）和单独行二尖瓣置换组（n=22）.对围术期相关疗效指标进行对比分析.二尖瓣置换均选用机械瓣膜,同期行三尖瓣成形术45例,左房血栓清除术10例.结果 两组患者无一例死亡,同期行房颤射频消融术组患者与单独行二尖瓣置换组患者相比较,体外循环时间及主动脉阻断时间较长（P〈0.05）,但呼吸机辅助呼吸时间、ICU逗留时间及术后住院时间无明显差异（P〉0.05）.随访6～21个月,无晚期死亡,同期行房颤射频消融术组患者中3例出现房颤复发,两组患者均未出现血栓栓塞事件;同期行房颤射频消融术组患者左心室射血分数及短轴缩短率较单独行二尖瓣置换组患者明显提高（P〈0.05）.结论 二尖瓣机械瓣膜置换同期施行房颤双极射频消融术可明显改善患者的左心功能,其对患者术后早期恢复并无负面影响.
