静息门控断层心肌灌注显像在冠心病治疗抉择中的价值 被引量:6

摘要 目的探讨静息门控断层心肌灌注显像在冠心病治疗抉择中的价值。方法 54例心脏疾病患者,其中冠心病32例,动脉粥样硬化13例,其他心脏疾病9例(心肌桥3例、心肌病5例、X综合征1例),均行静息门控断层心肌灌注显像检查,并与冠状动脉造影结果进行对比分析。结果静息门控断层心肌灌注显像阳性率分别为:冠心病组94%(30/32)、动脉粥样硬化组69%(9/13)、其他心脏疾病组100%(9/9)。结论静息门控断层心肌灌注显像可灵敏反映心肌血供状况及预估心肌活力,在心血管疾病,尤其是冠心病(CAD)的诊断、危险分层、疗效评估、预后判断和治疗方案的制定中有着非常重要的价值。应作为冠脉造影及支架安放或搭桥术前基础评估手段,避免盲目过度治疗。 Objective To investigate the value of resting gated myocardial perfusion imaging in coronary heart disease treat- ment choices. Methods Fifty-four cases with heart diseases including 32 cases with coronary heart disease, 13 cases with atherosclero- sis and 9 cases with other heart disease ( myocardial bridge in 3 cases, cardiomyopathy in 5 cases, 1 cases of X syndrome) underwent resting gated myocardial perfusion imaging examination. The results were compared with that of coronary angiography. Results The positive rate of resting gated myocardial perfusion imaging were 94% in the 6ibnarart disease patients (30/32) ,69% in the athero- sclerosis patients(9/13), and 100% in the other cardiac disease patients (9/9). Conclusion The resting gated myocardial perfusiori imaging can sensitively reflect the status of myocardial blood and myocardial viability. It has a great value in diagnosis, risk stratifica- tion, curative effect evaluation, prognosis and treatment plan for cardiovascular disease, especially coronary heart disease. It should be used as a basic evaluation method before coronary angiography and stent placement or bypass in order to avoid the blind excessive treat- ment.
