全身与局部用药在预防种植牙术后感染中的临床研究 被引量:18

Clinical effects of systemic medication and topical medication on prevention of infections after dental implants

摘要 目的比较分别采用全身用药和局部用药在预防种植牙术后发生感染的效果,为合理使用抗菌药物提供依据。方法选择自2008年8月-2013年8月因牙列缺损行牙种植术的患者共162例,随机分为全身组与局部组各81例,所有患者均采用ITI种植系统,全身组给予口服罗红霉素联合奥硝唑,局部组在种植体周围及切口内局部给予盐酸米诺环素联合甲硝唑,观察两组患者术后疼痛的发生情况以及用药效果。结果全身组术后完全无疼痛感的有41例占50.62%,局部组患者术后完全无疼痛感的有53例占65.43%,全身组术后完全无疼痛感的患者明显低于局部组(χ2=3.650,P<0.05);疼痛程度组间比较,局部组明显优于全身组(Z=-0.182,P<0.05);全身组的显效率为67.90%,局部组的显效率为85.19%,局部组的显效率明显高于全身组(χ2=6.739,P<0.05)。结论在牙种植术后采用局部应用盐酸米诺环素联合甲硝唑预防感染的效果优于全身用药,值得临床推广。 OBJECTIVE To compare the effects of systemic medication and topical medication on prevention of infections after dental implants so as to guide the reasonable use of antibiotics.METHODS Totally 162cases of dentition defect,who underwent the dental implants by using ITI implant system from Aug 2008 to Aug 2013,were enrolled in the study and randomly divided into the systemic medication group and the topical medication group with 81cases in each,then the systemic medication group was treated with oral administration of roxithromycin combined with ornidazole,while the topical medication group was given the topical administration of minocycline hydrochloride combined with metronidazole around the implant or inside the incision,and the incidence of postoperative pain and the effect of drug therapy were observed and compared between the two groups.RESULTS There were 41(50.62%)cases of patients without any postoperative pain in the systemic medication group and 53(65.43%)cases in the topical medication group(χ2=3.650,P0.05);as compared with the degree of pain,the topical medication group was significantly superior to the systemic medication group(Z=-0.182,P0.05).The effective rate of the topical medication group was 85.19%,significantly higher than 67.90% of the systemic medication group(χ2=6.739,P0.05).CONCLUSION The topical administration of minocycline hydrochloride combined with metronidazole is superior to the systemic medication on the prevention of infections after the dental implants,and it is worthy to be promoted in the hospital.
