摘要
目的:与病理对照,描述和分析肝腺瘤(HCA)的MR影像特征。方法:对15例病人共16个经手术病理证实的HCA资料行回顾性分析,包括病灶的大小,形态、平扫T1加权、T2加权信号强度及均匀性,有无含脂肪成分、出血、囊变坏死,动态增强的特征,有无假包膜,周围肝实质有无脂肪肝或肝硬化背景。结果:16个病灶大小2.5~29.0cm(平均10.3±7.5cm);13个(81.3%)病灶形态规则,呈圆形或椭圆形,3个病灶形态不规则,呈巨块伴分叶状改变;平扫显示4个病灶信号均匀,12个病灶信号不均匀,T1WI显示8个病灶为高信号,T2WI显示8个病灶为等或低信号;MR显示含脂质的病灶4个,MR显示出血病灶8个,囊变坏死10个,显示包膜征象10个,所有病灶均为富血供病变;所有病例周围肝实质均无肝硬化,2例有轻度脂肪肝。结论:在没有肝硬化背景基础上发生的富血供的信号不均匀肿块,且伴T1高信号和包膜是肝腺瘤一定特征的MR征象。
Purpose: Compared with histopathologic findings, to describe the magnetic resonance imaging features of hepatocellular adenoma (HCA). Methods: MR findings of 16 pathologically proved hepatocellular adenoma in 15 patients were retrospectively analyzed. Image analysis included the following: maximum size of lesion, contour (regular or lobulated), signal intensity (SI) and overall homogeneity on T1, T2-wcighted images, presence of region of steatosis, presence of macroscopic hemorrhagic component (defined as focal Tl-weighted hyperintense area), presence of necrotic or cystic component (defined as pronounced hyperintense signal on T2-weighted images) within the lesion, presence of pseudocapsule (low or high signal intensity on T2WI and delayed phase enhancement), dynamic enhancement pattern of lesion, presence of steatosis or cirrhosis in surrounding liver parenchyma. Results: The size was 2.5-29.0cm (mean 10.3~7.5cm) for all 16 lesions. The morphology of 13 lesions was regular with round or oval shape. Tumor heterogeneity was observed in 12 (75%) lesions. Hyperintensity on T1WI was observed in 8 (50%) adenomas; hypointensity or isointensity on T2WI was observed in 8 (50%) lesions. The steatosis component in 4 adenomas was shown by MRI. Presence of macroscopic hemorrhagic, necrotic or cystic component and pseudocapsule was observed in 8 (50%), 10(62.5%) and 10(62.5%) adenomas, respectively. All lesions were shown as hypervascular by MRI. The surrounding liver parenchyma had no cirrhosis in all cases and 2 patients had mild fatty liver. Conclusion: The hypervascular mass without cirrhosis background, associated with signal heterogeneity and hyperintensity on T1WI, presence of pseudocapsule, are common MR features of HCA.
