galectin-1与galectin-3在黑色素细胞病变中的表达及预后意义 被引量:3

Expression and prognostic significance of galectin-1 and galectin-3 in benign nevi and melanomas

摘要 目的 探讨galectin-1和galectin-3在不同黑色素细胞病变中的表达及其临床预后意义.方法 应用组织芯片及免疫组织化学EnVision法检测galectin-1和galectin-3在39例良性皮肤痣、58例皮肤原发恶性黑色素瘤、24例黏膜原发恶性黑色素瘤、77例转移性黑色素瘤中的表达.结果 galeetin-1和galeetin-3在良性皮肤痣中的表达均高于皮肤原发恶性黑色素瘤、黏膜原发恶性黑色素瘤及转移性黑色素瘤(均P<0.01).galectin-3在皮肤原发恶性黑色素瘤胞核中的表达高于黏膜原发恶性黑色素瘤(P<0.01)及转移性黑色素瘤(P<0.01),并与患者年龄、肿瘤组织坏死状态相关(P<0.05);生存及预后分析显示,Clark分级与皮肤原发恶性黑色素瘤患者总生存时间相关(P=0.037).galectin-3在所有黑色素瘤病例瘤细胞核内的表达水平与TNM分期是影响患者总生存时间的相关因素(P<0.01),但只有TNM分期是独立预后因素.结论 galectin-1和galectin-3在黑色素瘤中表达下降;galectin-3在胞核内的表达下降,可能预示黑色素瘤预后不良. Objective To study the expression and prognostic significance of galectin-1 and galectin-3 in different melanocytic lesions.Methods The expression of galectin-1 and galectin-3 in 39 cases of benign nevus,58 cases of primary cutaneous melanoma,24 cases of primary mucosal melanoma,69 cases of melanoma with lymph node metastasis and 8 cases of melanoma with distant metastasis were studied by immunohistochemistry and tissue microarray.Results The expression of galectin-1 and galectin-3 was higher in benign nevi than in melanomas (P 〈 0.01).The nuclear expression of galectin-3 was higher in primary cutaneous melanomas than in primary mucosal melanomas or melanomas with metastases (P 〈 0.01,respectively).The expression correlated with age of patients (P 〈0.05),necrosis (P 〈0.05) and survival time (P 〈0.01).Clark's level also correlated with survival time in patients with cutaneous melanomas (P =0.037).TNM staging was the only independent prognostic factor for melanomas (P 〈 0.01).Conclusions The expression of galectin-1 and galectin-3 is decreased in melanomas.The decrease in nuclear expression of galectin-3 may represent a poor prognostic factor for melanomas.TNM staging is an independent prognostic factor which influences the survival time.
