摘要
目的研究医院环境分离出的携带抗药基因的凝固酶阴性葡萄球菌(CNS)病原学及对乙醇消毒剂的抗性的研究,为更加有效的控制CNS所致的医院感染提供依据。方法采集医院环境表面和医护人员手标本841份,采用细菌分离鉴定、PCR和肉汤稀释法,对医院环境标本进行检测并测定携带抗药基因的CNS对乙醇消毒剂的抗力。结果从标本中分离出63株CNS,其中有12株携带qacA基因,检出率为19.05%;24株携带mecA基因,检出率为38.09%。药敏结果显示耐甲氧西林凝固酶阴性葡萄球菌具有多药耐药特征。乙醇消毒剂对12株携带qacA基因的CNS的MIC与MBC值有一株低于标准菌株,一株高于标准菌株,其余均与标准菌株相同。结论医院环境表面和医务人员手上检出的CNS呈现多药耐药性,并且83.33%携带抗药基因的菌株对乙醇消毒剂的抗性也已达到实验标准菌株水平。
Objective To study the etiology and the ethanol disinfectant resistance coagulase negative staphylococcus ( CNS ) from medical enviroment, in order to provide evidence und effective Prevent nosomial infection caused by CNS. Methods Collected 841 specimens of substance surface and medical personnel hand from medical environment, with the methods of bacteria separation and identification, PCR and broth dilution to test the specimens of hospital environment and determine the resistance of CNS carrying resistance genes to ethanol disinfectant. Results 63 plants CNS isolated from specimens, there are 12 plants carrying qacA gene, detection rate is 19.05%; 24 strains carry mecA gene, detection rate is 38.09%. Drug susceptibility results showed methicillin-resistant coagulase negative staphylococcus aureus has the characteristics of multi-drug resistance. There are 12 MIC and MBC values of Ethanol disinfectant to CNS carrying qacA gene, one of which is below standard strain, one is higher than the standard strains, the others are same as the standard strain.Conclusion CNS detected from substance surface of hospital environment and medical personnel hand presented multidrug resistance, and resistance to ethanol disinfectant of 83.33% strains carrying resistance genes is also up to the experimental experimental standard strains.
