摘要
目的研究胸椎旁间隙阻滞对血流动力学的影响及复合全身麻醉应用于开胸手术中的麻醉效果。方法２３例单侧 开胸手术病人分为两组，观察应用利多卡因－布比卡因（利布合剂）行胸椎旁间隙阻滞对血压、心率的影响，比较单次胸 椎旁间隙阻滞复合全身麻醉与硬膜外阻滞复合全身麻醉开胸手术中最低肺泡有效浓度（ＭＡＣ）及血压、心率等指标的变 化。结果（１）利布合剂用于胸椎旁间隙阻滞对血压、心率无明显影响；（２）两组病人术中各时间点吸入麻醉药的ＭＡＣ 值与血压、心率无明显差异。结论胸椎旁间隙阻滞对血流动力学影响轻微，复合全身麻醉应用于开胸手术可产生与硬 膜外复合全身麻醉相似的麻醉效果。
Objective To observe the effect of thoracic paravertebral space block with bupivacaine-lidocaine mixture solution on hemodynamics and the efficiency of combined general anesthesia in unilateral thoracotomy. Method Twenty three patients undergoing unilateral thoracotomy were divided into paravertebral space block (n=11) and epidural block (n=12) groups. Hemodynamic changes after thoracic paravertebral space block were recorded and the minimal alveolar anesthetic concentration(MAC), blood pressure, heart rate monitored during operation. Result Slight changes in the hemodynamics were observed after successful induction of paravertebral space, and the MAC, arterial pressure and heart rate did not show much difference between the two groups during operation. Conclusion Paravertebral space block with 0.375% bupivacaine-1% lidocaine mixture solution may induce slight changes in the hemodynamics and when combined with general anesthesia,anesthehc effects similar to that of epidural block in unilateral thoracotomy was achieved.
