小肝癌多层螺旋CT血供特点与微血管密度和血管内皮生长因子表达的相关性 被引量:1

摘要 目的探讨小肝癌(SHCC)的多层螺旋CT血供特点与微血管密度(MVD)、血管内皮生长因子(VEGF)表达的相关性。方法选择经手术病理证实的SHCC患者42例,根据SHCC血供的不同分型,采用免疫组织化学方法检测肿瘤组织MVD及VEGF的表达,分析各型SHCC的MVD及VEGF与SHCC血供的相关性。结果 42例SHCC患者中,肝动脉供血型29例(69.0%);肝动脉和门静脉双重供血型及少血供型各5例(各11.9%);门静脉供血型3例(7.1%);肝动脉供血型及双重供血型SHCC的MVD计数均高于少血供型,差异有统计学意义(P<0.05),肝动脉供血型与双重供血型MVD计数比较差异无统计学意义(P>0.05);肝动脉供血型、双重供血型、少血供型和门静脉供血型VEGF表达阳性率分别为31.0%、20.0%、80.0%和0.0%。肝动脉供血型和双重供血型、双重供血型和少血供型、肝动脉供血型和门静脉供血型之间VEGF阳性表达率比较差异均无统计学意义(P>0.05);肝动脉供血型和少血供型间VEGF阳性表达率比较差异有统计学意义(P<0.05),肝动脉供血型和双重供血型合并后与少血供型VEGF阳性表达率比较,差异有统计学意义(P<0.05)。结论 SHCC血供与MVD、VEGF具有相关性,多供血型(肝动脉供血型、双重供血型)SHCC组织中MVD高于少血供型,而VEGF表达阳性率则低于少血供型。 Objective To investigate the correlation between blood supply muhislice computerized tomography(MSCT) characteristics and microvessel density ( MVD ) , vascular endothelial growth factor( VEGF ) in small liver cancer ( SHCC ). Methods The MVD and VEGF expressions were examined by means of immunohistochemical technique in 42 cases of SHCC proved by pathology. All the cases of SHCC were divided into four teams according to different blood supply. The internal con- necting link between the blood form of SHCC and tumor tissue MVD, VEGF were analyzed respectively. Results Among the 42 cases of SHCC ,29 cases (69.0%) were supplied by hepatic arterial blood, 5 cases (11.9% )by arterial combine with portal blood supplying and poorly blood supplying respectively,3 cases (7.1% )portal blood supplying. The MVD count in the type of poorly blood supplying was lower than that in the other type of blood supplying( P 〈 0.05 ) ; there was no statistical differ- ence between hepatic arterial blood and portal blood supplying and poorly blood supplying( P 〉 O. 05 ). The VEGF expression rates were 31.0% ,20.0% ,80.0% and 0.0% corresponding in the type of hepatic arterial blood, portal blood supplying, poorly blood supplying and poorly blood supplying; there was no statistical difference among hepatic arterial blood, portal blood supplying and poorly blood supplying( P 〉 O. 05 ). There was statistical difference of VEGF expression between hepatic arterial blood and poorly blood supplying ( P 〈 0.05 ). After amalgamating the hepatic arterial blood and portal blood suppl- ying, there was statistical difference between the new team and poorly blood supplying ( P 〈 0.05 ). Conclusion There is cor- relation between blood type of SHCC and organization MVD, VEGF. MVD in the blood type (hepatic arterial blood type, double for blood type) for SHCC organization is higher than that of less blood type, while the VEGF positive expression is the opposite.
