摘要
目的:系统评价中医药治疗小儿厌食症的临床有效性和安全性。方法:采用Coehrane系统评价方法,检索1996—2011年的中国知识资源总库(CNKI)、维普数据库(VIP)和万方数据库(WMOD),同时手工检索相关的杂志及其他资料。对纳入的文献采用Cochrane协作网提供的RevMan5.1软件进行Meta分析。结果:Meta分析显示,①中医药治疗小儿厌食症临床疗效变化有统计学意义[RR=1.29[1.21,1.36](95%CI),P<0.00001],体重变化差异有统计学意义[SMD=2.30(0.93,3.67)(95%CI),P=0.0010],血红蛋白及发锌显示无统计学意义(P>0.05)。②安全性分析:因所纳入的文献只有2篇提到安全性,故无法得出安全性分析。③敏感度分析:因所纳入文献质量不高,故未做敏感性分析。结论:中医药治疗小儿厌食症有效,但由于纳入评价的文献质量不高,尚待更规范的临床随机对照试验,进一步证实其临床疗效。
Objective ：To approach the effectiveness and safety of traditional Chinese medicine （TCM） in treatment of children anorexia. Methods ： A comprehensive literature search was performed at CNKI, VIP database, WMOD for random- ized controlled trials. Hand search was also made to retrieve the current reference and research reports. The Coehrane Collaboration RevMan 5.1 software was used for Meta - analysis. Results ： （1）Meta - analysis showed that the clinical effi- cacy of TCM in treatment of children anorexia had significant difference [ RR = 1.29 [ 1.21,1.36 ] （95% CI）, P 〈 0. 00001 ], weight change difference was also statistically different [ SMD = 2.30 （0.93,3.67） （95 % CI） P = 0. 0010 ]. Hemoglobin and hair zinc indicated that the medical treatment group and the conventional Western treatment group had no significant difference（ P 〉 0.05 ）. （2）Security analysis：Only two studies mentioned security. It was unable to conclude the safety analysis. （3）The sensitivity analysis： All the studies in the review was low quality,so it was unsuitable to carry the sensitivity analysis. Conclusion：Despite the apparent reported positive findings, there is insufficient evidence to support ef- ficacy of TCM for children anorexia due to the poor methodological quality and the small number of trials of the included studies. Its curative effect still needs some large sample - size and well - design RCTs.
