摘要
目的应用监测-培训-计划(MTP)合理用药干预模式进行干预对照研究,调研我院外科围手术期预防性应用抗菌药的状况,为临床合理用药提供参考。方法分别选取我院2011年1～6月Ⅰ类切口手术出院病历100份作为对照组和2012年1～6月Ⅰ类切口手术出院病历100份作为干预组。对两组患者围手术期预防应用抗菌药物的选择、给药时机、住院药费、平均抗菌药物费用等的合理性进行比较分析。结果采用MTP模式实施干预后,我院Ⅰ类切口手术预防应用抗菌药物使用率由100%降至58%,联合用药比例由49%降至5%,品种选择合理率由30%升至90%,用药疗程合理率由15%升至89%;患者平均住院天数下降,平均抗菌药费用降低。结论采用MTP模式干预的方法能显著提高围手术期预防用抗菌药物的合理应用率,降低药物费用,对促进临床合理应用抗菌药物具有良好效果。
Objective To compare and analyze the usage of prophylactic antibacterial drugs during perioperative peri- od with the application of monitoring-training-planning （MTP） mode for intervention control study,and provide refer- ences for rational use of antibacterial drugs in clinic. Methods 100 cases of type I incision operations in our hospital from January to June in 2011 as control group and 100 others from January to June in 2012 as intervention group were collected. The rationalities of perioperative prophylactic application of antibacterial agents were compared between two groups in terms of medicine selection, administration time, hospitalization cost and the average cost of antibacterial agents, etc. Results With the intervention of MTP mode ,the percentage of prophylactic use of antibaeterials decreased form 100% to 58% ,the rate of combination use of drugs decreased from 49% to 5% ,the rate of reasonable choice of antibaeterials increased from 30% to 90% ,the rate of medication duration increased from 15% to 89%;The average hospitalization days decreased, the average cost of antibacterial agents decreased. Conclusion Application of MTP in- tervention mode could improve the rationality of use of antibacterial agents during perioperative period and reduce drug cost, which was effective for improving rational use of antibacterial agents.
