摘要
目的 :对 110 9例采用选择性射频热凝治疗三叉神经痛病例疗效进行分析讨论。方法 :仰卧位 ,Hartel前入路穿刺法 ,局部浸润麻醉 ,刺中三叉神经节后以温控射频热凝对靶点毁损。作者对此项技术作了部分改进。结果 :疗效优良 85 4例 ,良好 2 0 2例 ,无变化 5 3例 ,总有效率 95 .3%。结论 :认为改进后的方法简便、安全、有效 ,对老年人更为适宜。每次毁损参数以温度 6 0～ 75℃ ,热凝时间 2～ 4m
Objective:Selective percutaneous radiofrequency were performed in 1109 patients with trigeminal neuralgia,577 males and 532 females.Their ages ranged from 20 to 86 years old,average 52.Method:A special stereotaxic instrument was used for the target of procedure.This treatment was suitable for aged and weak patients who can not endure surgery.The temperature of tip was 70～80℃ and total heating time no more than 4 mins.These measures which have evolved from our experience in treating trigeminal neuralgia by this method over the last 10 years have made the procedure safer and less painful but also,significantly more expedient.Result:Excellent result was obtained in 854 cases(77%),improved in 202 cases (18.2%) and no change in 53 cases.Conclusion:No serious complications and no mortality were found in this group.The surgical indications,procedure,skill,electrtode positioning and neurolytic lesion are discussed in detaill
