摘要
报告 2 4例显微血管减压 (MVD)后复发的三叉神经痛 (TN)患者 ,分析其复发的原因主要是 :1责任血管错认、漏认 ;2减压材料选择或使用不当 ;3“新生”的责任血管 ;4减压区的炎症反应。首次提出对于术中未发现责任血管及三叉神经远端有血管压迫的患者 ,也应行三叉神经入脑区 (REZ)围套式减压或 MVD。
Twenty-four cases of recurrent trigeminal neuralgia(RTN)after microvascular decompression(MVD) were reported in this study.The main causes of RTN after MVD include the following several factors;First,the inability to identify the offending vessel may cause the early recurrence.Second,there were some errors in the selects or uses of the interpositors in MVD.The third,new offending vessels occured.Last,there were some inflammatory reactions in the area of MVD.It was reported for the first time that,distal decompression of the fifth cranial nerve combined with decompression at the root entry zone (REZ) should be also performed when the compression vessel was found in the distal portion of the trigminal nerve and the decompression at REZ should be also performed even if no offending vessel was found.
