血凝酶气管内注入联合机械通气治疗新生儿肺出血的临床疗效观察 被引量:10

Clinical efficacy of intratracheal instillation of hemocoagulase combined with mechanical ventilation in the treatment of neonatal pulmonary hemorrhage

摘要 目的:为了进一步提高新生儿肺出血的临床治疗效果,分析和探讨在进行机械通气辅助呼吸的同时进行气管内注入血凝酶的可靠性和安全性。方法:按照随机分配的原则将2009年1月-2013年1月在该院产科出生的肺出血患者62例分为观察组和对照组,其中对照组31例患者治疗上给予机械通气同时给予抗感染、纠正代谢紊乱、补充凝血因子、营养支持等一般治疗,而观察组31例患者在给予机械通气、抗感染、纠正代谢紊乱、补充凝血因子、营养支持等一般治疗同时通过呼吸机在肺表面出血部位局部喷洒血凝酶,记录和探讨两种不同治疗方案给患者带来的益处和可行性。结果:经过统计分析发现,观察组和对照组患者在死亡人数方面比较差异具有统计学意义(P〈0.05);观察组患者在肺止血时间和呼吸机脱机时间均显著少于对照组,两组比较差异同样具有统计学意义(P〈0.05)。结论:对于新生儿肺出血的患者在进行机械通气治疗的同时在气管内注入血凝酶,不仅可以显著减少患者的死亡率,而且有利于病情早期康复,值得临床推广。 Objective: To further improve the clinical treatment of neonatal pulmonary, analyze and discuss the reliability and se- curity of hemocoagulase intratracheal instillation during breathing with mechanical ventilation. Methods: In accordance with the principle of random , 62 patients with pulmonary hemorrhage treated in obstetric of our hospital from January 2009 to January 2013 were divided into ob- servation group and control group, and the control group (31 patients ) was treated with mechanical ventilation while giving other general treatments like anti- infection, correct metabolic disorders, supplementary coagulation factors and nutritional support, the observation group (31 patients ) was treated with breathing machine to spray hemocoagulase for the bleeding sites of lung surface while giving general treat- ments as above, the benefits and feasibility of two different treatment methods were recorded and investigated. Results : After statistical anal- ysis, the results showed that the deaths cases of observation group and control group reduced significantly, the differences of two groups were statistically significant ( P 〈 0. 05 ) ; the time of hemostasis and offline for lungs in observation group was significantly less than the control group, the differences were also statistically significant ( P 〈 0. 05 ) . Conclusion: For the treatment of neonatal pulmonary hemorrhage pa- tients , adopting intratracheal instillation hemocoagulase while giving mechanical ventilation therapy not only can reduce mortality significant- ly, but also conducive to the early rehabilitation of disease, which worthy of promotion in clinic.
