摘要
目的 探讨磁共振成像(MRI)在急性胆红素脑病(ABE)中的诊断价值.方法 选择2008年2月~2012年8月在广东省高州市人民医院就诊的60例病理性黄疸患儿作为研究对象,根据不同的血清胆红素水平分为A、B、C三组,对所有患儿进行MRI扫描,测定并比较三组患儿T1WI、T2WI和DWI扫描序列两侧苍白球的信号强度,并测定三组患儿双侧苍白球ADC、eADC值.结果 14例患儿双侧苍白球T1WI信号增强,60例患儿双侧苍白球T2WI和DWI信号强度肉眼观察无差异;B组患儿T1WI信号强度高于A组,C组患儿T1WI信号强度高于A组和B组,差异均有统计学意义(均P< 0.05),三组患儿双侧苍白球T1WI信号强度随着血清胆红素水平的增高而增高,同组患儿左侧和右侧苍白球比较,T1WI信号强度差异无统计学意义(P>0.05),三组患儿T2WI信号强度及双侧苍白球ADC、eADC值组间及组内比较差异均无统计学意义(均P> 0.05);7例确诊急性胆红素脑病患儿均位于C组,MRI表现为两侧苍白球T1WI呈对称性高信号,信号强度平均值左侧为(1186.65±62.34),右侧为(1192.47±63.38),高于同组平均数,差异均有统计学意义(均P< 0.05),T2WI和DWI信号强度无改变.结论 MRI扫描在ABE的诊断中有重要作用,两侧苍白球T1WI对称性高信号是ABE主要的MRI表现,信号强度随血清胆红素水平增高而增高,T1WI信号强度增高是临床诊断ABE重要的参考依据.
Objective To investigate the value of magnetic resonance imaging (MRI) in diagnosis of acute bilirubin encephalopathy (ABE) in neonatus.Methods 60 patients with pathologic jaundice in the People's Hospital of Gaozhou City from February 2008 to August 2012 as the research objects,and were divided into group A,B and C according to the level of serum bilirubin,all patients were scanned by MRI,and the signal intensity of both sides of the globus pallidus were compared and measured between the three groups in T1WI,T2WI and DWI sequences,and ADC,eADC value of the bilateral pallidal were measured.Results T1WI signals of bilateral pallidal of 14 children were enhanced,there were no differences of T2WI and DWI signal intensity in 60 children with the naked eye; T1WI signal intensity of group B was higher than that of group A,T1WI signal intensity of group C was higher than that of group A and B,the differences were statistically significant (all P 〈 0.05),the signal intensity of T1WI of the bilateral globus pallidus increased with the increasing of serum bilirubin level,compared the left and right globus pallidus in the same groups,the difference of T1WI signal intensity were no statistically significant (P 〉 0.05),T2WI signal intensity and the value of ADC,eADC of bilateral pallidal between groups and within groups had no statistically significant differences (all P 〉 0.05); 7 cases with acute bilirubin encephalopathy were located in the group C,MRI showed symmetrical high signal intensity of bilateral pallidal T1WI,the average value of signal intensity on the left was (1186.65±62.34) and on the right was (1192.47±63.38),they were higher than that of the same group,the differences were statistically significant (all P 〈 0.05),the T2WI and DWI signal intensity did not change.Conclusion MRI scan has important role in diagnosing ABE,on both sides of the globus pallidus,T1WI symmetrical high signal in MRI is the major expression,signal intensity increased with the increasing of serum bilirubin levels,it is an important reference for the clinical diagnosis of ABE that T1WI signal intensity increases.
