摘要
目的 比较和评估Cornell与Sokolow指数在老年女性高血压左室肥大（LVH）心电图诊断中的价值,探索提高心电图诊断效能的方法.方法 选择2011年3～12月间江苏省南通市妇幼保健院门诊检查的70例老年女性为研究对象,根据超声心动图测定的左室重量指数（LVMI）将其分为对照组20例,LVH组50例.计算Cornell及Sokolow指数的诊断灵敏度、特异度和准确率,并在不同特异度条件下比较Cornell和Sokolow指数诊断差异.结果 LVH组的LVM及LVMI的数值明显高于对照组,两组比较差异有统计学意义（P＜0.05）;Cornell指数的诊断灵敏度、特异度、准确度分别为75.23％、96.08％、87.58％,而Sokolow指数分别为60.35％、96.28％、72.78％,两者的特异度比较,差异无统计学差异（P＞ 0.05）;Cornell指数的诊断灵敏度和准确率较Sokolow指数高,两者相比差异有统计学（P＜0.05）.结论 与Sokolow指数比较,Cornell指数是诊断左室肥大较好的指标,对心电图诊断左室肥大有一定的指导意义.
Objective To compare and evaluate the diagnostic value by the Cornell index and Sokolow index for elderly female hypertensive subjects with left ventricular hypertrophy （LVH）,in order to find a more effective method to improve ECG diagnosis.Methods 70 elderly female who had health examination in Nantong Maternity and Child Health Care Hospital from March to December 2011 were selected as study objects,and they were divided into 20 cases in control group and 50 cases in LVH group according to the left ventricular mass index （LVMI） determined by echocardiogram.The sensitivity,specificity,and accuracy of Cornell index and Sokolow index were calculated.The difference between Cornell index and Sokolow index at different specificity were compared.Results The level of LVM and LVMI in LVH group were significantly higher than those in control group,the difference was statistically significant （P ＜ 0.05）; the Cornell index sensitivity,specificity,and accuracy were 75.23％,96.08％,and 87.58％ respectively,those in Sokolow index were 60.35％,96.28％,and 72.78％ respectively,no significant difference on the specificity for diagnosis of LVH between two indexes was observed （P ＞ 0.05）.The sensitivity and accuracy of Cornell index were higher than those of Sokolow index （P ＜ 0.05）.Conclusion Compareing with the Sokolow index,the Cornell index is a better indicator for elderly female hypertensive subjects with left ventricular hypertrophy.
