摘要
为探讨结肠灌洗在左半结肠癌致肠梗阻一期切除吻合术中的应用价值，回顾2012年3月至2013年4月我院行一期切除吻合术治疗、术中给予结肠灌洗的40例左半结肠癌致肠梗阻患者资料（观察组），片与同期于术前肠道准备下行一期切除吻合术治疗的40例非梗阻性左半结肠癌患者（对照组）进行对比分析。结果显示，两组患告均顺利完成手术，术后尤吻合门漏发生。观察组手术效果良好37例，欠佳3例（切口感染3例）；对照组手术效果良好38例，欠佳2例（切口感染1例。肠瘘1例）。两组患者手术效果比较差异无统计学意义，P〉0．05。观察组患者术后第2天、第3天肠蠕功频率明显快于对照组．P〈0．05；第4天两组差异无统计学意义，P〉0．05。结果表明．左半结肠癌致肠梗阻患者应用术中结畅灌洗法行一期切除吻合术可达到与非梗阻性左半结肠癌患者一期手术相同的疗效,证明在半结肠癌致肠梗阻一期切除吻合术中结肠灌洗的安全性与有效性，其应用价值值得肯定。
In order to explore the application value of colonic lavage in primary resection 8. anastomosis for left colon carcinoma associated with intestinal obstruction, authors retrospectively analyzed the data of 40 patients with this disease who were admitted to our hospital from March 2012 yr. to April 2013 yr. and had been subjected to primary resection ~. anastomosis,and to intraoperative colonic lavage （as observation group） ,and compared them with another 40 patients with non-obstructional left colon carcinoma who had been subjected to primary procedure after intestinal preparation（ as control group）. As results, on both groups the surgery was all performed smoothly, and without anastomosis stoma leakage; in observation group 37 cases were good,no-better in 3 cases（incision infection） ,in control group good in 38 cases,poor in 2 cases （one case,incision infection ; one cases,intestinal fistula） ;as to surgcial outcomes there was no sta- tistical difference（ P 〉0.05） ;on the 2nd and 3rd day after operation the enterocinesia frequency of obser- vation group was significantly faster than that of control group（ P-~C^0.05）,but on the 4th day the differ- ence had just no statistical significance （ P ,-~0.05）. Results show that for the patients with left colon carci- noma associated with intestinal obstruction in primary resection ~ anastomosis performing colon lavage can obtain the same therapeutic effect as non-obstructional left colon carcinoma, which thus confirmes the safety and effect of colonic lavage,and has certain application value.
