摘要
目的探讨非体外循环下冠状动脉搭桥术的临床效果。方法选取非体外循环下冠状动脉搭桥术53例作为观察组，另选取行传统的体外循环下冠状动脉搭桥术125例作为对照组，观察两组患者的手术时间、术中出血量、术中输血量、ICU留住时间、拔管时间、住院时间、住院总费用、并发症发生率。结果观察组手术时间、术中出血量、术中输血量分别为（3．78±0。65）h、（518．82±114．48）ml、（427．6t±153．18）ml，对照组分别为（7．62±0．83）h、（704．17±133．75）ml、（818．9±179．59）ml，两组差异均有统计学意义（t=11．025、13．256、10．287，均P〈0．05）。观察组ICU留住时间、拔管时间、住院时间、住院总费用分别为（23．82-i-10．49）h、（6．27±1．16）h、（10．71±2．30）d、（54683±4990）元，对照组分别为（82．71±8．26）h、（19．47±3．88）h、（15．84±1．97）d、（84358±5120）元，两组差异均有统计学意义（t=16．245、11．247、19．285、13．209，均P〈0．05）。观察组患者中有5例发生并发症，发生率为9．43％；对照组患者中发生并发症31例，发生率为34．8％；两组并发症发生率差异有统计学意义（x2=5．447，P=0．020）。结论非体外循环下冠状动脉搭桥术可降低对患者的损伤，术后心功能恢复快，并发症少，值得临床推广应用。
Objective To investigate the clinical effect of coronary artery bypass grafting without cardiopul- monary bypass. Methods 53 cases of coronary artery bypass grafting without cardiopulmonary bypass were selected as the observation group,while 125 cases of coronary artery bypass grafting with cardiopulmonary bypass were consid- ered as the control group. The operative time, blood loss, blood transfusion, ICU retain time, extubation time, length of hospital stay, total hospital costs and the incidence rate of complications were compared between the two groups. Results The operative time,blood loss,blood transfusion,ICU retain time,extubation, time,length of hospital stay ,to- tal hospital costs and the incidence rate of complications in the observation group were （3.78 ±0.65 ）h, （518.82 ± 114.48）m1,（427.61 ± 153.18）m1,（23.82 ±10.49） h, （6.27 ± 1.16） h, （10.71 ± 2.30） d, （54 683 ±4 990）, 9.43 %, those in the control group were （ 7.62 ± 0.83 ） h, （ 704.17 ±133.75 ） ml, （ 818.94 ± 179.59 ） ml, （ 82.71 ± 8.26）h, （19.47 ± 3.88）h, （15.84 ±1.97 ）d, （84 358 ± 5 120）,34.8%. The differences between the two groups were significant （ t = 11. 025,13. 256, 10. 287,16. 245,11. 247,19. 285,13. 209, X2 = 5. 447, all P 〈 0. 05 ）. Conclusion Coronary artery bypass grafting without cardiopulmonary bypass can reduce the damage to the patients, has faster recovery of postoperative cardiac function, fewer complications, which is worth to be applied in clinical.
