摘要
目的探讨中医手法复位联合椎弓根钉棒内固定术对脊柱胸腰段骨折的治疗效果。方法将100例脊柱胸腰段骨折患者随机分为对照组(49例)以及治疗组(51例),于治疗组患者,先行中医手法复位,再行椎弓根钉棒内固定术,对于对照组患者则直接采用椎弓根钉棒内固定术进行治疗。观察并比较两组患者术后临床疗效、Cobb’s角、椎体前缘及后缘高度、椎间盘退变程度及神经功能恢复情况。结果治疗组临床治愈率为66.67%,显著高于对照组的55.10%,具有统计学意义(P<0.05,χ2=9.053);两组患者治疗后Cobb’s角、椎体前缘及后缘高度均较治疗前显著改善(P<0.05);治疗后治疗组神经功能恢复情况及椎间盘退变情况均显著优于对照组,均具有统计学意义(P均<0.05)。结论中医手法复位联合椎弓根钉棒内固定术治疗脊柱胸腰段骨折疗效肯定,可促进患者术后神经功能及椎间盘恢复,疗效优于单用椎弓根钉棒内固定术治疗。
Objective To explore the clinical effect of manipulative reduction combined with transpedicular screw fixation on thoracolumbar spine fractures. Methods Total 100 patients with thoracolumbar spine fracture were randomly assigned into control group （49） and treatment group （51）. The control group received the internal fixation with transpedicular fixation system, while the treatment group received the manipulative reduction before internal fixation. The clinical effects, Coob＇ s angle, vertebral height of anterior and posterior borders,intervertebral disc degeneration and neural function recovery were observed and compared between the two groups before and after treatmentsResults Compared with control group, the curative ratio in treatment group increased significantly from 55.10% to 66.67% （ P 〈 0.05, X2 = 9. 053 ） ; Significantly recovery of Coob＇ s angle and vertebral height was observed in all patients after treatments（ P 〈 0.05 ） ;the intervertebral disc degeneration and neural function recovery were improved significantly in the treatment group than in the control group after treatments（ P 〈 0.05 ）. Conclusion There is a confirmed clinical effect of combination therapy with manipulative reduction and internal fixation on the thoracolumbar spine fractures. It can accelerate the recovery of neural function and intervertebral disc degeneration, and has a better effect as compared to internal fixation alone.
