摘要
目的 提高乳腺癌术后切口包扎的舒适度,降低皮下积液、皮瓣坏死的发生率.方法 将80例实施乳腺癌根治术的患者按照随机数字表法分为观察组（n=40）和对照组（n=40）,观察组使用固定胸带,对照组给予传统的绷带加压包扎.对两组患者之间舒适度;皮下积液、皮瓣坏死的发生率;包扎耗时、换药次数、引流量进行比较分析.结果 观察组患者中,舒适28例占70.0％,对照组舒适8例占20.0％,观察组舒适度优于对照组,差异有统计学意义（x2=20.20,P＜0.01）.观察组患者术后皮下积液、皮瓣坏死的发生率分别为5.0％,2.5％,明显低于对照组的22.5％,20.0％,差异有统计学意义（x2分别为5.156,6.135;P ＜0.05）.观察组患者包扎耗时、换药次数、总引流量分别为（153.51±5.99）s,（1.24±0.45）次,（123.33±7.57） ml,明显优于对照组的（301.13±15.17）s,（3.35±1.14）次,（203.67±14.84）ml,差异有统计学意义（t分别为15.678,2.986,5.194;P ＜0.01或0.05）.结论 自制固定胸带能够提高患者的舒适度,显著减少术后并发症,省时省力,经济实用.
Objective To improve the comfort of the wound dressing after radical operation of mastocarcinoma as well as reduce the incidence of subcutaneous fluid and skin flap necrosis.Methods Eighty cases of radical mastectomy patients were randomly divided into experimental group （n =40） and control group （n =40）.The experimental group used the fixed thoracic belt,while the control group was bandaged with a traditional bandage.The comfort level,incidence rate of subcutaneous fluid and skin flap necrosis,time-consuming of the bandage,dressing frequency and drainage were compared between two groups.Results Twenty eight cases of the experimental group felt comfortable,accounting for 70.0％ of the total.Only 8 patients of the control group felt comfortable,accounting for 20.0％ of the total.The comfort level of the experimental group was statistically significantly higher than the control group（x2 =20.20,P ＜0.01）.The incidence of subcutaneous fluid and skin flap necrosis of the experimental group was 5.0％ and 2.5％respectively,significantly lower than that of the control group which was 22.5％ and 20.0％,and the differences were statistically significant（x2 =5.156,6.135,respectively; P ＜ 0.05）.The time-consuming of the bandage,dressing frequency and drainage was respectively （153.51 ± 5.99） seconds,（1.24 ± 0.45） times and （123.33 ±7.57） ml in the experimental group,better than （301.13 ±15.17） seconds,（3.35 ±1.14）times and （203.67 ± 14.84） ml in the control group,and the differences were statistically significant （t =15.678,2.986,5.194,respectively; P ＜0.01 or 0.05）.Conclusions The fixed thoracic belt can improve patients＇ comfort significantly and reduce the incidence of postoperative complications,which is time-saving,economical and practical.
