摘要
目的 :探讨内皮素 1在高血压盐敏感者肾脏排钠障碍及肾脏损害中的作用。方法 :2 1例血压正常志愿者和 32例原发性高血压患者用急性静脉盐水负荷法确定盐敏感者 ,并观察 2 4小时及急性静脉盐水负荷 4小时内血、尿内皮素 - 1、尿钠和尿微白蛋白排泄量及动态血压水平。结果 :盐敏感者 ,无论血压正常或高血压患者 ,基础状态或盐水负荷后 ,尿内皮素 - 1排泄量均显著低于盐不敏感者 ,而血浆内皮素 - 1在各组间差异不著显。盐敏感性高血压患者急性静脉盐水负荷后 4小时内尿钠排泄量显著低于盐不敏感者。盐敏感性高血压患者尿微白蛋白排泄增加。尿内皮素 -1排泄量与尿钠排泄量呈正相关 ,而与尿微白蛋白排泄量呈现负相关。结论 :盐敏感者尿内皮素 - 1排泄减少 。
Aim:To compared the excretion of ET 1,urinary sodium,and microalbumin in salt sensitive hypertension patients and the healthy persons. Methods:21 cases of normotensive volunteers and 32 cases of hypertensive patients underwent acute saline loading test. We observed circulating and urinary endothelin\|1、urinary sodium and microalbumin excretion in 24 hours and 4 hours during acute saline loading period. Results:Compared with SR, SS showed decreased urinary endothelin\|1 excretion in normotensive group( \%P\%< 0 05) and hypertensive group( \%P\%< 0 01), regardless of saline loading or not, although there was no difference in plasma endothelin\|1. Urinary sodium within 4 hours of acute saline loading was lower in hypertensive SS than SR ( \%P\%< 0 05). 24 hours urinary microalbumin excretion was higher in SS than in SR patients ( \%P\%< 0 01). Urinary endothelin\|1 excretion was in positive correlation with urinary sodium and negative with urinary microalbumin excretion ( \%P\%< 0 05). Conclusion:Urinary endothelin\|1 excretion is decreased in SS, which may contribute to renal sodium retention and renal injury.
