摘要
目的探讨早期血清总免疫球蛋白E(IgE)水平在成人原发性肾病综合征(PNS)发病机制及诊断中的意义。方法纳入2010年1月至2012年12月期间浙江大学医学院附属第一医院收治的成人原发性肾病综合征患者194例(PNS组)和其它泌尿系统疾病患者(肾小球肾炎、狼疮性肾炎、过敏性紫癜肾炎()疾病对照组)134例,入院48 h内采集血标本,同时选取100例同期健康体检者作为健康对照组。检测各组血清总IgE、白蛋白(Alb)、肌酐(Cr)、血尿素氮(Bun)水平并比较其差异。结果与健康对照组及疾病对照组比较,PNS组Alb水平均较低(Z=-13.594,P<0.001;Z=-13.608,P<0.001),而总IgE水平均较高(Z=-3.155,P=0.002)。PNS组Cr、Bun水平均比健康对照组高(P均<0.05),而与疾病对照组无明显差异(P均>0.05)。此外,PNS患者血清总IgE水平虽远高于肾小球肾炎患者(Z=-3.393,P=0.001),但与狼疮性肾炎、过敏性紫癜肾炎患者差异不大(Z=-1.276,P=0.205;Z=-1.480,P=0.139)。结论早期血清总IgE可能与成人PNS发病机制有关,但它不能用于PNS的筛查或诊断。
Objective To investigate the significance of the early serum level of total IgE in pathogenesis and diagnosis of adult primary nephrotic syndrome (PNS). Methods A total of 328 patients, including 194 consecutive patients with adult PNS (PNS group) and 134 patients with other urinary diseases (glomerulonephritis, lupus nephritis and henoch-schonlein purpura nephritis) (disease control group) admitted in the First Affiliated Hospital, Zhejiang University School of Medicine from January 2010 to December 2012, and 100 healthy controls (healthy control group) were enrolled in the study. The serum levels of IgE, albumin (Alb), creatinine (Cr), blood urea nitrogen (Bun) in all the cases were measured within 48 hours of admission and compared between the groups. Results The level of Alb was significantly lower and the serum level of IgE was remarkably higher in the PNS group than in other groups (all P 〈 0.05). The serum levels of Cr and Bun in the PNS group were obviously higher than those in the healthy control group, but no significant differences were noted as compared to the disease control group(all P 〉 0.05). Furthermore, patients in the PNS group showed a higher serum level of IgE than the patients with glomerulonephritis (Z =-3.393, P = 0.001), but there were no statistical differences in the serum level of IgE between PNS patients, lupus nephritis patients (Z=-1.276, P = 0.205) and the patients with henoch-schonlein purpura nephritis (Z =-1.480,P = 0.139). Conclusion The early serum level of total IgE might have relationship with the pathogenesis of PNS, but it cannot be used as a biomarker for screening or diagnosis of PNS.
