摘要
为研究急性髓系白血病 (AML)细胞 P170表达与白血病细胞体外集落生长之间的关系及其预后意义 ,对 31例 AML 住院患者骨髓细胞用 Dicke法进行 AML 白血病集落 (AML- CFU)体外培养 ,并用单抗免疫组化法对骨髓细胞和 AML- CFU细胞 P170表达进行检测。 2 8例 AML 患者采用蒽环类药 + Ara- c治疗 ,2个疗程后评定疗效。结果显示 :初发、复发 AML、慢粒急粒变及 CR期 AML 患者 AML- CFU集落分别为 172 .9± 72 .1/ 2×10 5 ,176 .0± 76 .6 / 2× 10 5 ,2 2 4.5± 5 4.7/ 2× 10 5和 9.7± 5 .0 / 2× 10 5 ,初、复发 AML 及 CML 急粒变患者 AML-CFU明显高于 CR期 AML 患者 (P<0 .0 1) ,而初、复发 AML 及慢粒急粒变患者 AML- CFU无差异。 AML 患者AML- CFU细胞和 BM细胞 P170阳性表达和 P170阴性表达均为 16例。两种细胞 P170阳性表达一致者 13例 ,P170阴性表达一致者 12例 ,P170阳性和 P170阴性表达不一致者各 3例 (P=0 .114)。 P170阴性者 CR10例 (10 /13,76 .9% ) ,P170阳性者 11例 CR2例 (2 / 11,18.2 % ) ,P=0 .0 0 48。多元回归及逐步回归分析 AML 患者 CR率与P170表达及疾病状态有关 ,其相关系数分别为 - 0 .9786和 - 0 .40 5 5 (P值分别为 0 .0 0 94和 0 .0 10 5 )。以上可见 ,AML-
This study was aimed at the relationship between P170 expression and colony formation unit leukemia (CFU L) in acute myeloid leukemia(AML), and their clinical significance. The expressions of P170 on leukemia cells from bone marrow (BM) and CFU L of 31 hospitalized cases with AML (including 15 newly diagnosed cases, 9 relapsed cases, 4 CML BC transformed cases, and 3 cases in CR) were examined by means of immunocytochemistry using the monoclonal antibody(JSB 1). CFU L assays were performed simultaneously. The results demonstrated that the CFU L numbers in 15 newly diagnosed cases, 9 relapsed cases, 4 CML BC transformed cases and 3 cases in CR were 172.9±87.0/2×10 5, 176.0±76.6/2×10 5,224.5±54.7/2×10 5 and 9 7±5.0/2×10 5, respectively. The number of CFU L was lower in CR cases than in the other three groups ( P <0 01). Both of P170 + and P170 - expressions on cells of CFU L and BM were observed in 16 cases. The P170 + and P170 - expression coincidence for the cells of CFU L and that for BM were observed in 13 cases and 12 cases, respectively; while the P170 + and P170 - expression unconformity for the cells of CFU L and that for BM were 3 cases and 3 cases( P =0.114), respectively. The CR rates of induction chemotherapy were 2/11(18.2%) for P170 + and 10/13(76.9%) for P170 -; the differences between them were significant ( P =0.0048). The results of multiple linear regression and stepwise regression analyses demonstrated that the chemotherapy response of patients with AML was not related to sex, age, WBC, the percentage of blasts in peripheral blood and BM, the cellularity of BM, and the number of CFU L, but it was related to P170 expression and disease status. The regression coefficients were -0.9786 and -0.4055, the P values were 0.0094 and 0.0105, respectively. These suggest that the greater number of CFU L in AML is associated with the active status and no remission of the disease. P170 expression is an independent prognostic factor, and the P170 + expression indicates a poor prognosis.
