负性协同刺激分子PD-L1在重症肌无力患者外周血的表达及意义

摘要 目的研究重症肌无力(MG)患者外周血负性协同刺激分子程序性死亡因子1(PD-1)及配体(PD-L1)的表达,探讨其与MG发病机制的关系。方法采用免疫荧光标记、流式细胞术检测67例MG患者和50例健康对照者外周血单个核细胞中PD-1和PD-L1的表达,用ELISA法检测各组血浆中可溶性PD-L1(sPD-L1)的水平。结果 MG组外周血中CD4+PD-1+T细胞数多于对照组;MG组外周血中CD14+单核细胞PD-L1表达高于对照组;CD4+T细胞上PD-1的表达在MG组并发胸腺瘤的高于未并发胸腺瘤的患者;CD14+单核细胞上PD-L1的表达在MG组并发胸腺瘤的也高于无胸腺瘤的患者。MG组患者血清中sPD-L1含量低于对照组;血清sPD-L1的浓度与年龄、性别、患病类型等无相关性。结论 PD-1/PD-L1信号途径参与了MG的病理过程。 Objective To explore the relationship between the negative co-inhibitor programmed death ligand-1 (PD-L1) and the pathogenesis of myasthenia gravis (MG). Methods Peripheral blood samples were collected from 67 MG patients(group MG) and 50 healthy persons (group C). The expressions of programmed death-1 (PD-1) and PD-L1 on peripheral blood mononuclear cells (PBMC) were detected using immune-fluorescence labeling and flow cytometry assay,and the concentrations of soluble PD-L1 (sPD-L1) in plasma were measured using ELISA kit. Results The quantity of CD4+ PD-1+ T cells was higher in group MG than that in group C. The expression of PI)-L1 on CD1"4+ mononuclear cell was higher in group MG than that in group C. The expression of PD-1 on CD4+ T cell and that of PD-Lq on CD14+ cell were higher in MG patients with thymoma than those without tyrnorna. Plasma concentration of sPD-L1 was lower in group MG than that in group C. The expression of sPD-L1 was not significantly correlated to age, gender and type of disease. Conclusion The PD-1/PD-L1 signal pathway participates in the pathological process of MG.
