慢性阻塞性肺疾病急性加重期患者血尿酸肌酐比与疾病严重程度相关性研究 被引量:7

Corralative study on the level of serum uric acid to creatinine ratio in patients with acute exacerbated chronic obstructive pulmonary disease

摘要 目的探讨慢性阻塞性肺疾病急性发作期(AECOPD)患者病情严重程度与血尿酸肌酐比、肺功能及慢性阻塞性肺疾病评估测试(CAT)评分的相关性。方法选取我科因AECOPD住院患者共198名,收集患者临床资料、动脉血气分析、肾功能、肺功能检查指标及CAT评分,并进行分析。结果患者血尿酸肌酐比与氧合指数、FEV1实/预%、FVC实/预%存在明显负相关,相关系数r分别为-0.301,-0.312,-0.331,P值均<0.05。肺功能越差,缺氧程度越重,血尿酸肌酐比升高越明显。而CAT分值与血尿酸肌酐比、肺功能及缺氧程度也有明显相关性。结论血尿酸肌酐比可作为AECOPD严重程度的一项重要参考指标。CAT作为评估疾病对患者影响的简单量表,能较快速准确地为临床医生制定治疗方案提供依据,值得临床推广使用。 Objective To study the level of serum uric acid to creatinine ratio (UA/Cr)in patients with acute exacerbated chronic obstructive pulmonary disease (AECOPD),and its relations with lung function,oxygenation index and the score of COPD assessment test(CAT).Method 198 patients were enrolled,and their clinical dates,renal function,lung function,arterial blood gas,and the score of CAT were collected.Result The significant negative relationship were shown in patients with AECOPD,and the UA/Cr correlated with oxygenation index (r =-0.301),with FEV1 % (r =-0.312),with FVC% (r =-0.331),P 〈 0.05.The worse lung function,the lower oxygen level,the higher level of UA/Cr.The score of CAT has significant corralativity with lung function and oxygenation index.Conclusion The UA/Cr is an additional parameter for evaluating the severity of AECOPD.As a simiple scale to evaluate the influence of COPD,CAT could supply evidence to make treatment rapidly and accurately,and worthy of being widely applied in clinic.
