摘要
目的对3个部位视网膜表面膜进行组织结构比较性研究。方法孔源性视网膜脱离或其术后病人的黄斑表面膜、星状皱襞和前部视网膜环形表面膜通过玻璃体切除手术获得,分离的膜组织按层次分为近玻璃体一面的膜和紧贴视网膜面的膜。应用3种抗细胞外间质(ECM)抗体:Ⅰ型胶原、Ⅲ型胶原和纤维粘连蛋白(FN)进行膜组织免疫电镜研究。结果在黄斑表面膜和星状皱襞膜中,靠近玻璃体一面的膜组织以胶原为主,分布的细胞是上皮样细胞;在近视网膜面的膜组织中,细胞的数量和类型都明显增多。环形膜组织成分主要是Ⅰ型胶原和有色素颗粒的上皮样细胞。在黄斑表面膜和星状皱襞膜组织中足突细胞的分布层次上有差异。结论3种视网膜表面膜组织结构不完全相同,这种特征与膜发生时间无关,而玻璃体后界膜的位置可能影响视网膜表面膜的组织结构。星状皱襞膜收缩中心较黄斑表面膜的收缩中心贴近膜组织表面。
ObjectiveTo compare the ultrast ructural characterisitics of epimacular membranes,star folds and circumferential membranes.MethodsAn immuno electron micr oscopic study was performed on the epire tinal membranes from three different loc ations. All specimens were surgically removed by vitrectomy in patients w ith rhegmatogenous retinal detachment as sociated with Proliferative vitreoretinopactiy (PVR).The membrane specimens were divided into two groups,those near the vitreous and those near the retina based on the level of the peeling membranes.The immun ostaining antibodies for extracellular m atrix(ECM) included type Ⅰ collagen,type Ⅲ collagen and firbronectin(FN).ResultsThe specimens near the vitreous in epimacu lar membranes and star folds contained a moderate number of epithelial like cell s and abundant collagen deposits.In comparison,the number and the shape of cells were obv iously increased in specimens near the re tina.Circumferential membranes were main ly composed of type Ⅰ collagen and epith elial like cells with pigment.In additio n,cells with foot processes surround ed by abundant collagen fibrils were loc ated in the specimens near the vitreous in s tar folds,and in epimacular membranes,ho wever,similar cells were also found in sp ecimens near the retina.ConclusionOur stud y suggests that three different local ep iretinal membranes show a histology diff erence.The ultrastructural study of thes e membranes may further elucidate the mechan ism that determine location and morphol ogy of epiretinal membranes.The working hypothesis is that position of posterior vitreous detachment is probably related to shape and location of epiretinal mem branes,and the duration of membrane form ation has less influence.The contraction center of star folds is more superficial than that of epimacular membranes.
